自体表皮移植治疗白癜风120例临床观察及其影响因素分析

摘　　要：目的评估白癜风行自体表皮移植术治疗后的长期，临床疗效并分析其相关影响因素。方法采用薄层削片自体表皮移植术治疗白癜风，通过复诊、随访观察不同型别白癜风的长期疗效，并分析其与移植后时间长短及患者性别、年龄、治疗部位的相关性。结果接受自体表皮移植资料完整的稳定期白癜风患者120例，寻常型86例（男52例，女34例），节段型34例（男14例，女20例），共移植皮片828个，随访3个月～3年后的总有效率为76．33％，寻常型与节段型有效率分别为70．59％和82．90％。移植部位色素减退15例，原位复发7例，其他部位新发5例。对可能影响疗效的相关因素分析显示，自体表皮移植治疗白癜风后1年内尚稳定，随时间延长，疗效稍降低；移植疗效与白癜风型别、部位及患者性别、年龄有关（P〈0．05）。结论自体表皮移植治疗白癜风的长期疗效较好，患者性别、年龄及白癜风型别、移植部位、稳定期及病程长短为影响疗效的因素。Objectives To evaluate the long-term outcome of autologous epidermal grafting for vitiligo and its influen- cing factors. Methods 120 patients with vitiligo were treated with autologous epidermal grafting, and fol- lowed up by clinical revisit and interview. The associations of long-term efficacy of the treatment with the time after treatment,age,gender and body site were analyzed. Results Of the 120 patients,86 （52 were males and 34 females） had vitiligo vulgaris, and 34 （14 males and 20 females）had segmental vitiligo. A total of 828 epidermal sheets were transplanted. The total effective rate was 76.30%. The effective rates for vitiligo vulgaris and segmental vitiligo were 70.59% and 82.90% respectively. 15 patients developed hypopigmenta- tion in the recipient area. Relapse in the recipient areas developed in 7 patients, and new lesions occurred in 5 patients. The autologous epidermal grafting remained effective for one year after the therapy. Afterward the efficacy declined over time. The efficacy was related to gender, age, the type of vitiligo and body site（ P 〈 0. 05）. Conclusions Autologous epidermal grafting has long-term benefit for vitiligo. Gender,age, the type of vitiligo, transplanted body site, stable status and disease duration may be the important factors affecting the efficacy.
