摘 要:目的探讨磁共振T1ρ成像评价膝关节软骨早期退变的临床价值。方法对17例有右膝关节疼痛史的中国男子曲棍球队运动员(运动员组)及50名青年健康志愿者(无症状组),行膝关节T1ρ成像,生成T1ρ图,将髌软骨分为上、中、下层,每层又分为外侧、偏外侧、中间、偏内侧及内侧,共15个ROI,测量T1ρ值;并进行对比分析。结果 T1ρ成像均清晰显示两组共134个膝关节,运动员组双侧膝关节髌软骨相应ROI的T1ρ值均明显高于无症状组(P均<0.05)。运动员组右侧膝关节髌软骨各ROI的T1ρ值均高于左侧膝关节(P均<0.05)。无症状组左、右侧膝关节间,中层外侧(t=2.426,P=0.019)、中层内侧(t=2.549,P=0.014)、下层内侧(t=2.173,P=0.035)髌软骨T1ρ值差异有统计学意义,余12个ROI的T1ρ值差异均无统计学意义(P均>0.05)。结论 T1ρ值可敏感地量化反映膝关节髌软骨形态改变之前的早期退变。Objective To investigate the clinical value of Tlp MRI in assessing early articular cartilage degeneration in knees. Methods T1ρ images of the knees in 17 professional hockey players with the symptom of right knee pain (athlete group) and 50 asymptomatic young subjects (asymptomatic group) were obtained with 3.0T MR scanner. T1ρ pseudo col- or maps of patella cartilage were generated by post-processing. Each patella cartilage was divided into upper, central and lower layers, and the layers were partitioned to lateral, partial lateral, middle, partial medial and medial parts. Totally 15 ROIs were set and the Tlρ value of each ROI was measured and compared with each other. Results Tlρ images of patellar cartilage in all of 134 knees were clear. Tlρ values of bilateral patellar cartilage in athlete group were significantly higher than those in asymptomatic group (all P〈0.05). In athlete group, Tlp values of right patellar cartilage were obviously higher than left patellar cartilage corresponding ROIs. In asymptomatic group, there was no statistical difference of T1ρ values between the left and right patellar cartilages (all P〉0.05), except in the ROIs of central-lateral (t= 2. 426, P= 0. 019), central-medial (t=2. 549, P=0. 014) and lower-medial (t=2. 173, P=0. 035) parts. Conclusion The minimal changes of early articular cartilage degeneration in knees prior to the morphologic alterations can be quantitatively reflected by T1ρ values.
