摘  要:目的探讨引起老年患者院内下呼吸道感染的肺炎克雷伯菌中质粒介导的Amp C酶基因型,为产Amp C酶细菌的监测和防治提供依据。方法采用头孢西丁纸片对产Amp C酶菌株进行初筛;双纸片表型确证法确证产Amp C酶;采用UNIQ-10柱式质粒小量抽提试剂盒提取细菌质粒;采用聚合酶链反应(PCR)和DNA测序检测分析质粒Amp C酶基因型。结果双纸片表型确证试验检出17株产Amp C酶菌株,检出率为17.5%。经PCR扩增17株菌株均在405 bp处出现阳性条带,经DNA测序证实为DHA-1型质粒Amp C酶。结论浙江省临海市中医院存在质粒介导Amp C酶流行菌株,质粒Amp C酶主要以DHA-1型为主。Objective To understand the Amp C β-lactamases genotype of the plasmids mediated in Klebsiella pneumoniae causing nosocomial lower respiratory tract infection among elder patients and provide evidence for the surveillance of the bacteria producing Amp C β-lactamases and the prevention and control of related infection.Methods Cefoxitin disk diffusion test was conducted to detect the phenotype of Amp C β-lactamases,and Amp C β-lactamases was confirmed by dual paper phenotypic method.The plasmids was extracted from bacterium with minipreps kit UNIQ-10.Polymerase chain reaction(PCR) and DNA sequencing were conducted to detect the genotype of Amp C β-lactamase of the plasmids,Results A total of 17 K.pneumoniae strains producing Amp C β-lactamase genotype DHA-1 were detected,accounting for 17.5%.Conclusion Klebsiella pneumoniae producing plasmid-mediated Amp C β-lactamase circulated in Linhai County Traditional Chinese Medicine Hospital,and the plasmid-mediated Amp C β-lactamase mainly belonged to genotype DHA-1.
