试带法尿蛋白阴性而镜下管型异常的探讨

摘　　要：目的探讨干化学试带法尿蛋白阴性且沉渣镜检管型异常标本的疾病类型以及高血压和糖尿病早期肾损伤的检查。方法对33例试带法尿蛋白阴性标本进行湿化学法检测及沉渣镜检。结果 33份标本湿化学法均显示尿蛋白阳性,沉渣镜检管型异常。这些标本中高血压患者居多,占42.4%。5例患者Cys C超过正常。结论试带法尿蛋白检测阴性的标本,特别是高血压及糖尿病病人的标本,最好同时做湿化学法比对,每份标本均应做尿沉渣镜检。尿微量白蛋白可以作为检测早期肾损伤的有效手段。Objective The aim of this study was to discuss the diseases in the samples of negative urine protein by reagent strip with abnormal cast under microscope, as well as incipient nephropathy inspection of diabetes and hypertension. Methods Wet chemical method and microscope were used to examine 33 cases of negative urine protein by reagent strip. Results Thirty-three specimens＇ urinary protein all were tested positive by wet chemical method and the microscopic cast was abnormal. In these cases, hypertension patients were the majority for about 42.4%. Cys C was out of range in five patients. Conclusion For negative urine protein samples,especially the specimens from hypertension and diabetes patients, it is better to perform wet chemical method and microscopic examination for sure. Microalbuminuria is an effective assay for the detection of nephropathy.
