骨振器检测位置对骨导听性脑干反应结果的影响

摘  要:目的研究骨导听性脑干反应(Audi tory Brain stem Response,ABR)检测中,骨振器放置在不同位置时对测试结果产生的影响。探讨骨导ABR在临床听功能检测中的应用价值及研究方向。方法选择听力正常成年人,将骨振器分别放置在乳突部、耳后及额部正中,进行骨气导ABR检测,对检测结果统计分析。结果各部位骨导与气导ABR阈值间比较,差异有统计学意义(P<0.05)。各部位骨导间ABR阈值比较,差异无统计学意义(P>0.05)。在阈上30 d Bn HL刺激强度下,各部位骨导与气导ABR间Ⅰ、Ⅲ、Ⅴ波峰潜伏期(Peak Latency,PL)比较,差异均有统计学意义(P<0.05)。不同部位骨导间Ⅰ、Ⅲ、Ⅴ波PL比较,额部与耳后间Ⅴ波PL比较差异有统计学意义(P<0.05)。气导与各部位骨导以及各部位骨导间Ⅰ-Ⅲ、Ⅰ-Ⅴ波峰间期(Interpeak Latency,IPL)比较,差异均无统计学意义(P>0.05)。结论骨振器放置位置对声波的衰减极小,骨振器放置在不同位置时对骨导ABR波ⅤPL结果能够产生影响,检测时骨振器放于耳后相对更可靠。Objective The aim of this study was to observe the effects of different bone-oscillatory placement on the bone-conduct auditory brainstem response(ABR). There was significant value on studying the clinical application of bone-conduct ABR.Methods Bone-oscillatory was placed on the mastoid portion, back of the ear and the frontal placement in a group of normal hearing adults. The results of air and bone-conduct ABR were recorded and analyzed. Results There were significant differences for the threshold of both air-conduct and bone-conduct ABR testing in different placements(P〈0.05). There was no difference for the threshold of bone-conduct ABR testing in different placements(P〉0.05). There were significant differences(P〈0.05) for the latencies of each wave between air-conduct and bone-conduct ABR under 30 d Bn HL above their thresholds in different placements.There were significant differences for peak latency of Ⅴwave between the frontal placement and back of the ear(P〈0.05). There was no difference for I-Ⅲ and I-Ⅴ interpeak latency(P〉0.05). Conclusion Bone-oscillatory placement had minimal sound attenuation and affected peak latency of bone-conduct ABR V wave. It is reliable for the bone-oscillatory to be placed on the back of the ear.
