摘 要:目的分析妊娠期糖尿病(gestational diabetes mellitus,GDM)及孕母胰岛素应用与新生儿低血糖发生的相关性。方法选取2010年6月至2013年12月东莞市妇幼保健院GDM产妇分娩的新生儿82例作为研究组,同期糖耐量正常的产妇分娩的新生儿50例作为对照组。研究组分为研究组A(未应用胰岛素者,42例)和研究组B(应用胰岛素者,40例)。在新生儿出生后0.5 h、2 h、6 h、12 h、24 h取足后跟血进行微量血糖测定,并比较3组结果。结果研究组A发生低血糖症5例(11.90%),其中2例(4.76%)为巨大儿;研究组B发生低血糖症4例(10.0%),其中2例(5.0%)为巨大儿;对照组未出现低血糖症新生儿和巨大儿;3组比较差异有统计学意义(P<0.05);出生后0.5 h时对照组血糖值[(3.91±0.52)mmol/L]高于研究组A[(3.42±0.61)mmol/L]、研究组B[(3.41±0.62)mmol/L](P<0.05);出生后2 h、6 h、12 h、24 h 4个时间点3组新生儿血糖值差异均无统计学意义(P>0.05)。结论 GDM及胰岛素应用的产妇分娩的新生儿尤其是巨大儿易发生低血糖症。Objective To analyze the relativity of puerperas with gestational diabetes mellitus(GDM) and insulin application with neonatal hypoglycemia.Methods The 82 neonates who were given birthed by puerperas with GDM in DongGuan Maternal and Child Health Hospital from June 2010 to December 2013 were selected as study group, 50 neonates who were birthed by normal puerperas were selected as control group.The patients in study group were divided into study group A and study group B, 42 cases in group A who were given birthed by puerperas with GDM without using insulin, 40 cases in study group B who were given birthed by puerperas with GDM and insulin application.The foot heel blood of neonates 0.5 h, 2 h, 6 h, 12 h, 24 h after birth were taken to test blood glucose, the results were compared between 3 groups.Results There were 5 cases in study group A occurred hypoglycemia (11.9 %), including 2 cases of macrosomia(4.76 %);4 cases in study group B occurred hypoglycemia(10.0 %), including 2 cases of macrosomia(5.0 %);no hypoglycemia and macrosomia occurred in control group, the differences between three groups has significance (P〈0.05).The blood glucose level of neonates had no significant differences between 3 groups in 2 h, 6 h, 12 h, 24 h after birth (P〉0.05), The difference in blood glucose level of neonates between 3 groups[(3.42 ±0.61)mmol/L, (3.41 ±0.62)mmol/L,(3.91 ±0.52)mmol/L respectively] in 0.5 h after birth had statistical significance(P〈0.05).Conclusion The neonates who are born by puerperas with GDM and insulin application especially macrosomia are prone to be hypoglycemia.
