摘  要:目的比较本实验室已有的3种流感病毒检测方法的差异,以确定它们在不同情形下的适用方向。方法通过实时荧光定量反转录PCR、细胞培养和鸡胚培养3种方法同时检测488份流感样病例(ILI)鼻咽拭子样本,计算出它们的灵敏度和特异度,并进行统计分析。结果实时荧光定量RT-PCR、细胞培养和鸡胚培养的阳性检测率分别为16.80%、6.76%、3.07%;相对于实时荧光定量RT-PCR,细胞培养的灵敏度为40.24%,特异度为100.00%,鸡胚培养的灵敏度为18.29%,特异度为100.00%;与细胞培养相比,鸡胚培养的灵敏度为45.45%,特异度为100.00%。结论3种方法在灵敏度方面差异很大,但在特异性上没有差异。由于实时荧光定量RT-PCR具有很高的灵敏度,并且检测速度快,适用于暴发疫情的应急检测;细胞培养和鸡胚培养作为经典的病毒分离方法,可以获得流感毒株,用来开展抗原性、基因特性和耐药性等深入研究。Objective To compare the differences among three influenza virus detection assays being used in local laboratory, in order to determine their appropriate application fields. Methods 488 throat swabs samples from influenza - like illness (ILI) cases were detected by real -time fluorescent quantitative RT- PCR, cell culture and chicken embryo culture, then the sensi- tivity and specificity of these methods were calculated and assessed by statistics. Results The positive ratios of real - time fluo- rescent quantitative RT- PCR, cell culture and chicken embryo culture were 16.80% , 6.76% , 3.07% respectively. In con- trast to real -time fluorescent quantitative RT -PCR, the sensitivity and specificity of cell culture were 40.24% and 100% re- spectively, while those of chicken embryo culture were 18.29% and 100.00%. Chicken embryo culture showed sensitivity and specificity of 45.45% and 100.00% compared with cell cuhure. Conclusion There was significant difference in sensitivity but almost no in specificity among the three methods. With high sensitivity and fast testing velocity, the real -time fluorescent quantitative RT - PCR is suitable for rapid diagnosis in epidemic outbreak. Cell culture and chicken embryo culture, as a classi- cal method for etiological surveillance, can isolate influenza virus and conduct intensive studies on antigenicity, genetic charac- teristics and drug resistance.
