摘  要:目的了解2013年珠海市手足口病病原构成和流行病学规律。方法收集2013年珠海市手足口病临床诊断病例粪便标本215份,采用荧光RT-PCR和巢式RT-PCR进行病原学检测。结果 215份病例中,183例肠道病毒阳性,其中EV71阳性45例(24.59%)、Cox A16阳性8例(4.37%)、非EV71/非Cox A16的其他肠道病毒阳性130例(71.04%)。130例其他肠道病毒阳性标本中,113例的序列测定结果 Cox A6有93例(50.82%),Cox A10有9例(4.92%),Cox A2、Cox A5、Cox A4、Cox A20、Cox A14和Echo9各有少部分病例。8个血清型与各原型株之间的遗传距离较远,但每个型别的毒株都有高度相似的国内株存在。Cox A6和EV71检出高峰分别在9月和5月;Cox A6和EV71病例间平均年龄差异有统计学意义(P<0.05);Cox A6、EV71、Cox A10和Cox A16病例间男女比例差异无统计学意义(P=0.54)。结论 2013年珠海市手足口病前4位病原体依次为Cox A6、EV71、Cox A10和Cox A16。Cox A6和EV71发病有较明显的季节性,Cox A6组的平均年龄<EV71组。Objective To study the etiology and epidemiological characteristics of hand - foot - mouth disease (HFMD) in Zhu- hai, Guangdong province, in 2013. Methods A total of 215 clinical diagnosis of HFMD specimens were collected from Zhuhai in 2013, and etiology was detected by real - time RT - PCR and nested RT - PCR. Results There were 183 EV infections in 215 clinical diagnosis of HFMD specimens, including 45 EVT1 infections(24.59% ), 8 CoxA16 infections(4.37% ) and 130 non-EV71 non- CoxA16 EV infections (71.04%). Among 113 of the 130 ones, 93 were CoxA6 infections(50.82% ), 9 were CoxA10 infections(4.92% ). The remaining 11 cases were CoxA2, CoxAS, CoxA4, CoxA20, CoxA14 and Echo9 infec- tions. The genetic distance between 8 serotypes and the prototype strains was far, but each serotype strain was highly similar with domestic strains. The incidence peak of CoxA6 and EV71 were September and May, accordingly. The difference of average age between CoxA6 cases and EV71 cases was statistically different (P 〈 0.05), and the difference of gender distribution among CoxA6, EV71, CoxA10 and CoxA16 cases was no statistical difference (P 〈 0.05 ). Conclusion The first four pathogens of HFMD were CoxA6, EV71, CoxA10 and CoxA16 in Zhuhai in 2013. The CoxA6 and EV71 cases showed a clear seasonal. The average age of CoxA6 cases was less than EV71 cases.
