自控静脉镇痛法在阻塞性睡眠呼吸暂停低通气综合征手术后的应用

摘　　要：目的:探讨自控静脉镇痛法对阻塞性睡眠呼吸暂停低通气综合征患者术后镇痛的效果和安全性影响。方法:接受改良腭咽成形术的阻塞性睡眠呼吸暂停低通气综合征术后患者60例,随机分为实验组和对照组,每组30例,实验组术后给予自控静脉镇痛,对照组镇痛给予尼美舒利0.1g口服。对两组患者术后进行疼痛的测量及评估,观察患者术后的应激反应。结果:实验组与对照组比较,术后8,24h时,VAS评分低于对照组(P<0.05),简化麦吉尔评分,术后8,24,48h均低于对照组(P<0.05),术后8h血糖低于对照组(P<0.05)。结论:自控静脉镇痛法对阻塞性睡眠呼吸暂停低通气综合征患者术后镇痛有效、安全。Objective：To explore the efficacy and safety of patient-controlled intravenous analgesia on postoperative patients of obstructive sleep apnea/hypopnea syndrome（OSAHS）.Methods：60cases of OSAHS who underwent modified uvulopalatopharyngoplasty（UPPP）were randomly divided into study group and control group（n=30in each group）.The patients in the study group were applied with patient-controlled intravenous analgesia,while the control group with oral administration of 0.1g nimesulide.Then the postoperative pain scores were measured using the visual analogue scale and short-form McGill pain questionnaire.And the postoperative stress response was also observed and compared between the two groups.Results：The patients in study group had higher VAS scores after postoperative 8hand 24 hthan that in control group（P〈0.05）.The McGill scores at 8,24 and 48hwere lower than that in control group（P〈0.05）.Meanwhile,the blood sugar levels reduced in the study group at 8hafter operation（P〈0.05）.Conclusion：The patientcontrolled intravenous analgesia is effective and safety in postoperative analgesia for postoperativepatients with OSAHS.
