针刺结合苓桂术甘汤对椎管内麻醉术后尿潴留患者的影响

摘　　要：目的：探究中医针刺结合苓桂术甘汤治疗椎管内麻醉术后尿潴留患者的临床疗效,为临床尿潴留的治疗提供一种安全、有效、方便的方法。方法：选取自2009年1月-2014年5月来我院泌尿外科椎管内麻醉术后尿潴留患者102例作为研究对象,随机分为对照组和研究组,对照组43例（42.16%）采用西药治疗,选择臀大肌注射甲硫酸新斯的明注射液1 mg,2次/d,共2 mg,研究组59例（57.84%）采用中医方法 （针刺结合苓桂术甘汤）治疗,治疗结束后,观察并分析2组患者临床疗效、第1次排尿时间和残留尿量以及尿潴留等级分级。结果：对照组总有效率为79.07%（34/43）,研究组总有效率为94.52%（56/59）,存在统计学差异（χ^2=6.016,P=0.014〈0.05）,在第1次排尿时间和尿残余量上,两组患者存在统计学差异（t=2.339,8.474、P=0.021,0.000〈0.05）,在尿潴留程度分级上,2组患者存在差异（Z=-2.145,P=0.032〈0.05）。结论：针刺结合苓桂术甘汤治疗椎管内麻醉术后尿潴留患者效果显著,治疗时间短,残余尿量少,可以作为一种治疗方法。Objective： To explore the Chinese medicine acupuncture combined with the clinical curative effect of Ling-gui-zhugan decoction in the treatment of patients With urinary retention after intraspinal anesthesia of, provide a safe, effective and convenient for clinical treatment of urinary retention. Methods： 102 patients of intraspinal anesthesia postoperative urinary retention were selected as the research objects at urinary surgery in our hospital from January 2009 to May 2014, randomly divided into the control group and the research group, the control group of 43 cases （42. 16% ） treated with western medicine, Ming injection LMG gluteus maximus muscle injection of neostigmine methyl sulfate, two times a day 2 mg, the research group of 59 eases （57. 84% ） by the method of Chinese medicine （acupuncture combined with Lingguizhugan Decoction） treatment, after treatment, to observe and analyze the clin- ical efficacy of the two groups, the first mieturition time and residual urine volume and urihaty retention grade. Results ： In the control group, the total effective rate was 79. 07% （34/43）, the research group, the total effecfi~,e rate was 94. 52% （56/59）, and there was statistical difference （X^2 = 6. 016, P = 0. 014 〈 0. 05）, in the first micturition time and urinary residual volume, significant difference exists in patients of two groups （ t = 2. 339, 8. 474, P = 0. 021, 0. 000 〈 0. 05 ） , in the degree classification of urinary retention, two groups of patients are different （Z =-2. 145, P = 0. 032 〈 0. 05 ） , Conclusion： Acupuncture combined with the effect of Ling Gui Zhu Gan Decoction in the treatment of patients with urinary retention after intraspinal anesthesia of significant, short treatment time, residual urine volume is less, can be used as a method of treatment.
