参附注射液对创伤性脑疝大骨瓣减压后血压和颅内压的影响

摘　　要：目的:探讨不同剂量参附注射液对创伤性脑疝大骨瓣减压后外周动脉收缩压、舒张压以及颅内压的影响及用药安全性。方法:68例创伤性脑疝患者随机分为A、B、C 3组;A组23例使用小剂量参附注射液50m L+生理盐水250m L;B组23例使用大剂量参附注射液100m L+生理盐水250m;C组对照组22例仅使用生理盐水250m L。均在开颅钻第1个骨孔时静脉滴注,记录钻第1孔时及去骨瓣后15分钟2个时间点收缩压、舒张压和颅内压的数值并进行分析。结果:减压后A、B2组收缩压、舒张压均明显高于C组,差异具有统计学意义(P<0.05);减压后B组收缩压明显高于A组(P<0.05),但2组间舒张压无显著差异(P>0.05),A、B组颅内压与C组三组比较统计学无显著差异(P>0.05)。结论:参附注射液能维持创伤性脑疝大骨瓣减压后血压的稳定,大剂量对动脉收缩压的作用更明显,不同剂量对颅内压没有不利影响。Objective： To observe the effect and safety of different dosage of Shen-fu injection on blood pressure and in- tracranial pressure after large decompressive craniectomy in treating traumatic encephalocele. Methods： 86 traumatic encephalocele patients were distribute into three groups （ A, B and C ） and received large decompressive craniectomy.23 patients as Group A were got low dosage （ 50mL ） of Shen-fu injection. Other 23 patients as Group B were recieved high dosage （ 100mL ）of Shen-fu injection.The remaining 22 patients named Group C were as control group. The data of systol- ic blood pressure, diastolic blood pressure, and intracranial pressure were taken down at the time of first key hole drilling and craniectomy decompression15 minutes later. Results： After decompression the SBP and DBP of Group A and Group B were significant higher than Group C（ P〈0.05 ）. The SBP of Group B were significant higher than that of Group A（ P〈O.05 ）, and there was no significant difference of DBP between Group A and Group B （P〉0.05）. No significant difference had find among the ICP of Group A , B and Group C （ P〉0.05 ）. Conclusion ： Shen-fu injection can keep the blood pressure stable after large decompressive craniectomy in treating traumatic encephalocele.Low and high dosage of Shen-fu injection show no harmfulness to ICP, but high dosage get more powerful function to SBP.
