深圳市528例HIV-1感染者的分子流行病学特征

摘　　要：目的研究深圳市HIV-1病毒亚型、传播途径的分布特征,为艾滋病的整体防控工作提供帮助。方法采集528例HIV-1病毒感染阳性病例的流行病学信息及标本,运用逆转录巢式聚合酶链反应方法对病毒基因进行扩增、测序,确定基因亚型,并进行统计分析。结果 528例HIV-1感染者中86.6%为性传播途径感染。528份样本中共发现CRF01_AE、CRF07_BC、CRF08_BC、CRF02_AG等4种毒株及B、B’、C、A1等4种亚型,其中以CRF01_AE、CRF07_BC、B为主要流行毒株或亚型,其构成情况为54.5%(288/528)、25.0%(132/528)、9.3%(49/528)。CRF01_AE在男男同性恋感染者、异性性途径感染者、吸毒感染者的构成比分别为51.2%、59.3%、66.0%。结论深圳市HIV-1感染者以性传播途径为主。病毒型别呈现多样性特征,性传播、吸毒感染人群中以CRF01_AE为主要流行毒株,血液途径感染人群以B亚型为主要流行毒株。Objective To explore the epidemiological features of distribution of subtypes and transmission of HIV- 1 in Shenzhen and provide effective evidence for overall prevention and control of the disease. Methods The epidemiological data and samples of 528 HIV- 1 cases were collected. Viral genes of the HIV- 1 cases were amplified and sequenced by nested reverse transcription polymerase chain reaction and statistically analyzed. Results Among the 528 cases ,86.6% were infected through sexual contact. Four recombinant viral strains of CRF01_AE, CRF07_BC, CRF08_BC, CRF02_AG and four subtypes of B, B＇, C, A1 were found ,among which CRF01_AE, CRF07_BC, B were the predominant strains or subtypes, accounting for 54.6% （288/528）, 25.0% （132/528）, 9.3% （49/528）, respectively. The constituent ratio of CRFOI_AE in the MSM, heterosexual sexually transmitted patients, infected drug users were 51.2% , 59.3% and 66.0% ,respectively. Conclusion Sexual transmission was the main route of transmission of HIV- 1 infection in Shenzhen. HIV- 1 showed a diversity of virus types in Shenzhen. CRF01_AE was the major epidemic strain in sexually transmitted group and drug abuse crowd, whereas subtype B was predominant in the blood transmitted crowd.
