血清endocan质量浓度与急性心肌梗死的相关性研究

摘　　要：目的：检测急性心肌梗死患者血清endocan质量浓度并评价其与急性心肌梗死发病的关系。方法：对126例急性心肌梗死患者、108例稳定型心绞痛患者和82例健康对照者,采用酶联免疫吸附（ELISA）法测定各组的血清endocan质量浓度,通过Logistic回归分析评价endocan与心肌梗死发病的独立相关性,采取相关分析评价endocan与急性心肌梗死患者各临床特征的关系。结果：急性心肌梗死组患者的血清endocan质量浓度较稳定型心绞痛组及健康对照组显著升高（均P〈0.01）。Logistic回归分析结果显示,高血清endocan质量浓度与急性心肌梗死发病呈独立正相关（优势比1.49,95%可信区间1.11~1.70,P〈0.01）。血清endocan质量浓度与高敏C-反应蛋白（hs-CRP）呈正相关（r=0.26,P〈0.01）,而与肌酸激酶同工酶（CK-MB）无相关性（r=0.07,P〉0.05）。结论：高血清endocan质量浓度是心肌梗死发生的独立危险因子,endocan可能是预测心肌梗死发生风险的重要的生物学标志物。Objective： To detect serum endocan levels in acute myocardial infarction （AMI） patients and to investigate the relationship between endocan levels and the presence of AMI. Methods： One hundred and twentysix AMI patients, 108stable angina pectoris （SAP） patients and 82 controls patients were enrolled. Serum endocan levels were measured by enzyme-linked immunosorbent assay （ELISA）. The association between serum endocan levels and the presence of AMI was investigated via logistic analysis. The correlation between serum endocan levels and other clinical characteristics in AMI patients was assessed by correlation coefficient. Results： Serum endocan levels were significantly higher in patients with AMI compared to SAP patients or controls （ P 〈 0.01 for both）. Multivariate logistic regression demonstrated that increased serum endocan levels were independently associated with the presence of AMI in coronary artery disease（CAD） patients（P 〈 0.01 ）. Serum adropin levels were associated with high- sensitivity C- reactive protein （ hs- CRP） （ r = 0.26, P 〈 0.01 ） but not creatine kinase MB isoenzyme （ CK- MB） （ r = 0.07, P 〉 0.05 ） in AMI patients. Conclusion ： Endocan in serum was an independent risk factor for the presence of AMI. Endocan might be a promising biomarker in predicting AMI onset in CAD patients.
