四维牵引配合骶管封闭治疗腰椎椎管狭窄症32例

摘　　要：目的通过临床观察,验证四维牵引配合骶管封闭对腰椎管狭窄症的临床疗效。方法收集2013年8月—2015年2月期间我院门诊及住院患者,筛选出纳入标准的病人64例随机分为两组。治疗组应用四维牵引配合骶管封闭,对照组用手法配合骶管封闭。观察两组治疗前和治疗四周结束后根据JOA、Prolo等评分法及Epstein评定标准情况来制定评定标准,并进行总有效率的计算。观察其疗效情况。结果治疗后的JOA、Prolo评分及Epstein评定标准治疗组疗效明显高于对照组,18个月共观察治疗组32例患者,优4例（12.5%）,良12例（37.5%）,改善13例（40.6%）,差3例,总有效率为90.6%。愈后随访0.6～1.0年内无复发。统计学分析表明疼痛,功能活动等观察指标均具有统计学意义。结论四维牵引配合骶管封闭治疗方案安全性高费用少,并发症少,是治疗腰椎椎管狭窄的理想方法之一。Objective To test and verify the clinical curative effect of four-dimensional traction combined with sacral epidural block therapy on lumbar spinal stenosis. Methods According to the selection criteria, 64 cases in outpatient of our hospital from August 2013 to February 2015 were randomly diveded into treatment group and control group. 32 cases in the treatment group were treated with four-dimensional traction combined with sacral epidural block therapy. While the cases in the control group were treated with massage combined with sacral epidural block therapy. According to the evaluation criteria of JOA grades and Prolo grades after four weeks＇ treatment, the curetive effect was observed. Resules After four-week treatment, the JOA grades and Prolo grades of the treatment group are significantly high than those of the control group. In the 32 cases, 4 cases（12.5%） had remarkable curative effect, 12 cases（37.5%） had remarkable effect, 13 cases（40.6%） had effect, but 3 cases were ineffective, and the total effective rate reached 90.6%.No recurrence occured during the 0.6～1.0 year followup. The statistical analysis showed that the indexes including pain, function and activity of the observation group had statistical significance. Conclusion The four-dimensional tow combined with traction combined with sacral epidural block therapy had high safety, performance price ratio and few complications, so this therapy was one of the perfect treatment methods of lumbar spinal stenosis.
