自拟方治疗癌痛患者应用阿片类药物所致便秘

摘　　要：目的观察自拟经验方治疗癌性疼痛患者应用阿片类药物引起便秘的临床疗效。方法将212例符合纳入标准的患者,随机分为治疗组108例,给予自拟经验方(肉苁蓉、大黄、厚朴、生地黄等)治疗,对照组104例,给予麻仁软胶囊,2周为1疗程。观察治疗后疼痛评分、阿片类药物用量、肠鸣音、卡氏评分、GIQLI评估情况等。结果治疗组以气滞为主,肠道气滞、阴虚肠燥、脾肾阳虚为前3位证型,占71.70%,肠道实热证型占比例最小。治疗后疼痛评分、阿片类药物用量、肠鸣音、卡氏评分、GIQLI评估方面与对照组比较,差异有统计学意义(P<0.05)。结论自拟经验方治疗阿片类药物引起的便秘,能增加肠道动力,增加肠鸣音,改善便秘,减轻疼痛程度,提高患者生活质量。Objective To observe the therapeutic effect of constipation caused by opioid drug in the treatment of cancer pain patients. Methods The 212 cases of patient who meet the inclusion criteria, were randomly divided into treatment group 108 cases, given self experience prescription（cistanche 3, rhubarb, magnolia officinalis and radix rehmanniae, etc. ） ;the con- trol group 104 cases, give March soft capsule for 2 weeks, then clinical treatment. Results Treatment group with deficiency syndrome type, specific aspects in the qi stagnation, qi stagnation, yin deficiency of intestinal intestinal dryness, Yang defi- ciency of spleen and kidney as the top three cardtype accounted for 71.70%, the smallest proportion of intestinal excess heat syndrome type areaccounted for in the case; The experimental group after treatment, pain, opioid consumption, bowel sounds, card comparison group＇s score, GIQLI evaluation and control, the difference was statistically significant（ P 〈 0.05）. Conclu- sion The application of self experience prescription for the treatment of opioid induced constipation have security, and may increase the bowel sounds by improving the increased intestinal motility, improve defecate to remove the application limits of opioids, reduce the degree of pain, in order to improve the quality of life.
