洗必泰对机械通气患者口腔菌落的影响

摘　　要：目的观察洗必泰口腔护理液对机械通气(MV)患者口腔菌落的影响。方法将需经口气管插管行MV治疗的患者80例,随机分为治疗组及对照组,各40例,分别采用0.12%洗必泰溶液及0.9%生理盐水进行口腔护理,2组均每天进行常规口腔护理擦洗3次,取2组插管前、插管后3、6、9、12 h及插管后8、16、24 h口腔护理前、后30 min腭弓两侧、咽、扁桃体分泌物进行菌落培养,对比2组咽拭子菌落计数。结果 2组插管后3、6 h口腔菌落数显著低于插管前(P<0.05);治疗组护理后各时间点咽拭子培养菌落数显著低于对照组(P<0.05)。结论洗必泰口腔护理溶液可有效降低MV患者口腔部细菌数量,提高口腔清洁度,对预防呼吸机相关性肺炎(VAP)起积极作用。Objective To observe the effect of chlorhexidine oral care solution on the oral bacterial colonies in the patients on mechanical ventilation （MV） .Methods A total of 80 patients on MV through oral tracheal intubation and randomized into the treatment group and the control group with 40 cases in each group. The chlorhexidine solution （0.12%） and normal saline （0.9 % ） were used for oral care for the patients in the treatment group and the control group, respectively. Routine o- ral care three times every day was performed for the patients in the two groups. The secretions located in the bilateral palatine arch, pharynx, and tonsil before intubation, 3,6,9,12h after intubation, 8,16,24 h after intubation and before oral care, and 30 min after oral care in the two groups were used for bacterial colony cultivation. The throat swab colony count in the two groups was compared. Results The comparison of the number of oral colony 3,6 h after intubation was significantly lower than that before intubation （ P 〈 0.05）. The colony count in each timing point after care in the treatment was significantly lower than that in the control group （ P 〈 0.05）. Conclusion The chlorhexidine oral care solution can effectively reduce the oral colony count in the MV patients, enhance the oral cleanliness, and plays an active role in the prevention of ventila- tor-associated pneumonia（ VAP ）.
