摘 要:目的观察盐酸戊乙奎醚对多发伤致急性肺损伤(ALI)患者血浆中肺表面活性物质相关蛋白D(SP-D)、肿瘤坏死因子α(TNF-α)和白细胞介素8(IL-8)水平的影响及其临床疗效。方法选择多发伤致急性肺损伤患者40例,随机分成两组,实验组和对照组,每组20例。对照组给予常规标准治疗,实验组在对照组的基础上每12 h给予盐酸戊乙奎醚(长托宁,PHC)2 mg,持续7 d,分别于给药前、给药第3天和给药第7天,比较患者在SP-D、TNF-α、IL-8及氧合指数(Pa O2/Fi O)2、肺损伤评分(LIS)的变化及差异。结果与治疗前比较,实验组在给药第3天和第7天,患者血浆中SP-D、TNF-α和IL-8水平明显下降(P<0.05),并且Pa O2/Fi O2明显上升,LIS明显降低(P<0.05);而对照组改变不明显(P>0.05)。与对照组比较,在各时间点,实验组的各项指标均明显改善,差异有统计学意义(P<0.05)。结论盐酸戊乙奎醚能抑制炎症反应,减轻肺组织的损伤,改善氧合,能有效缓解多发伤导致的急性肺损伤。[ Objective ] To observe the efficacy of penehyelidine hydroehloride on the plasma levels of surfactant protein D (SP-D), tumor necrosis factor-α (TNF-α) and interlenkin-8 (IL-8) in patients with acute lung injury (ALI) induced by multiple trauma and discuss its clinical significance. [ Methods ] Forty cases with acute lung injury (ALl) induced by multiple trauma were randomly divided into experimental group (20 cases) and control group (20 cases). The control group received routine standard treatment alone and the experimental group were given additional pene- hyelidine hydrochloride (PHC) 2 mg on the basis of routine standard treatment every 12 hours for 7 days. The blood samples were collected and oxygen indexs (PaOJFiO2), lung injury score (LIS) were respectively recorded at the be- ginning of the PHC administration, 3 days after PHC administration and 7 days after drug administration to compare the change and difference in SP-D, TNF-α, IL-8, PaO2/FiO2 and LIS. [ Results ] Compared with the beginning, 3 days and 7 days after PHC administration, the levels of SP-D, TNF-α IL-8 and LIS in experimental group were de- creased significantly (P 〈 0.05); but PaO2/FiO2 was significantly improved (P 〈 0.05). Then compared with control group, the parameters mentioned above in experimental group at each time point were markedly improved and the differences were of statistically significance (P 〈 0.05). [ Conclusions ] Penehyclidine hydrochloride can inhibit in-flammatory reaction, reduce lung tissue injury, improve the arterial oxygenation and alleviate multiple trauma-in- duced acute lung injury effectively.
