摘 要:目的 观察吸入不同时限七氟醚对全麻手术患者罗库溴铵肌松效应的影响.方法 72例全麻手术女性乳腺病患者,随机分为4组(n=18):吸入七氟醚0 min组(S0组);吸入七氟醚20 min组(S20组);吸入七氟醚45 min组(S45组);丙泊酚组(P组).S0、S20、S45组分别在气管插管并吸入七氟醚0、20、45 min,P组在气管插管后,静脉注射罗库溴铵0.6 mg/kg,观察4组的起效时间、高峰时间、临床作用时间、恢复指数、体内作用时间、4次成串刺激(TOF)恢复至90%的时间.结果 与P组比较,S0组起效时间、高峰时间、临床作用时间差异无统计学意义(P>0.05);吸入七氟醚20 min后,随吸入时间延长,起效时间逐渐缩短,S0[(106±56)s]>S20[(77±23)s]>S45[(63±22)s](P <0.05),高峰时间逐渐延长,S0[(23.1 ±6.1)min] <S20[(27.7±4.2)min] <S45[(34.7±4.1) min] (P<0.05),临床作用时间逐渐延长,S0[(34.2±4.3)min] <S20[(38.7±2.3)min] <S45[(47.9 ±3.8)min](P<0.05);恢复指数延长,P[(10.6±2.2)min] <S0[(14.0±6.7) min]、S20[(15.1±1.9)min]<S45[(23.6±3.9)min] (P< 0.05),TOF90%恢复时间延长,P[(52.0±3.8) min] <S0[(62.7±3.8) min]、S20[(63.4±3.6)min] <S45[(82.4±4.2)min] (P<0.05);随吸入时间延长体内作用时间逐渐延长,P[(43.3±3.4) min] <S0[(49.9±4.9)min]< S20[(54.8±4.7) min] <S45[(75.2±3.5)min] (P<0.05).结论 七氟醚对罗库溴铵具有增效作用,其增强效应随吸入时间延长而增强,两药合用时应注意随吸入七氟醚时间延长,减少罗库溴铵用量.Objective To observe the influence of different time sevoflurane on rocuronium effect of muscle relaxants in patients with general anesthesia.Methods Seventy-two ASA Ⅰ-Ⅱ female patients undergoing breast operation with general anesthesia were randomly divided into four groups (n =18 each),inhaled 0 min sevoflurane group (S0 group),inhaled 20 min sevoflurane group (S20 group),inhaled 45 min sevoflurane group (S45 group),propofol group (P group).A bolus of rocuronium 0.6 mg/kg was given during maintenance of anesthesia.Neuromuscular function was monitored using train of four (TOF) response measured by accelemgraphy.The onset time,peak time,clinical role time,recovery index,response in vivo time,and the time of TOF recovery to 90% were recorded.Results The onset time and peak time showed no significant difference (P > 0.05) between S0 group and P group.Twenty min after inhalation of sevoflurane,with the inhalation time over,the onset time was gradually shortened:S0 [(106 ± 56) s] > S20 [(77 ± 23) s] > S45 [(63 ± 22) s] (P < 0.05);peak time was gradually extended:S0[(23.1 ±6.1) min] <S20[(27.7±4.2) min] <S45[(34.7±4.1) min] (P<0.05);clinical role time was gradually extended:S0 [(34.2 ± 4.3) min] < S20 [(38.7 ± 2.3) min] < S45 [(47.9 ± 3.8) min] (P<0.05);The recovery index was extended,P [(10.6 ±2.2) min]< S0[(14.0 ± 6.7) min],and S20 [(15.1 ± 1.9) min] < S45 [(23.6 ± 3.9) min] (P < 0.05);The TOF90% recovery time was extended:P [(52.0 ± 3.8) min] < S0 [(62.7 ± 3.8) min],and S20 [(63.4 ± 3.6) min] < S45 [(82.4±4.2) min] (P<0.05);The response in vivo time was gradually extended:P [(43.3 ± 3.4) min] <S0[(49.9±4.9) min] <S20[(54.8±4.7) min] <S45[(75.2±3.5) min] (P<0.05).Conclusion Sevoflurane can enhance the effects of rocuronium time-dependently.The dose of rocuronium should be reduced gradually with inhaled s
