显微镜手术联合麒麟丸治疗精索静脉曲张致弱精子症

摘　　要：目的 探讨显徽镜手术联合麒麟丸治疗精索静脉曲张（VC）合并的临床疗效.方法 80例精索静脉曲张合并弱精症患者,随机分成两组,单纯显微镜手术组40例,手术联合麒麟丸组40例.结果 80例患者均手术成功,两组术后症状消失,术后两组精液常规和血液激素有明显改善,精子密度、精子量、精子活力（a＋b级）明显提高、畸形精子百分率明显降低、术后的血清、精浆中的血清抑制素B（Inh B）、睾酮（T）含量明显改善,促卵泡激素（FSH）、黄体生成素（LH）含量降低,与术前比较,差异有统计学意义（P＜0.01）,手术联合药物组[精子密度（22.46±2.58）×106/L、精子活力（a＋b级）（60.43±3.14）％、Inh B（139.37 ±34.94） ng/L]比单纯手术组[精子密度（19.68±2.04）×106/L、精子活力（a＋b级）（53.42±3.97）％、Inh B（128.75 ±6.94） ng/L]改善效果更佳（P＜0.05）,术后随访8 ～12个月,手术联合药物组受孕率为77.50％（31/40例）,比单纯手术组受孕率55.00％（22/40例）效果明显（P＜0.05）.结论 显微镜手术联合麒麟丸治疗VC,对于术后的精子质量改善明显,疗效满意.Objective To explore the therapeutic effectiveness of microsurgical varicocelectomy and Qilin pill for varicocele complicated with asthenospermia,Methods Eigthy cases of varicocele with asthenospermia were randomly divided into two groups：simple surgical group （n =40）,and surgical treatment and Qilin pill （combined） group （n =40）.Results The microsurgical varicocelectomy was performed successfully on all patients,and the symptoms of varicocele disappeared.The semen quantity and the serum inhibin B （Inh B） were obviously improved （P ＜0.01） after operation in two groups.The semen density [（22.46 ±2.58） × 106/L] and vigour （60.43 ±3.14）％,and Inh B [（139.37 ±34.94） ng/L] were significantly increased in combined group as compared with simple surgical group [semen density （19.68 ± 2.04） × 106/L and vigour （53.42 ± 3.97） ％,Inh B （128.75 ± 6.94） ng/L,P ＜ 0.05].Pregnancy rate （77.50％） was significantly higher than in smple surgical group （55.00％,P ＜ 0.05）.Conclusion Microsurgical varicocelectomy and Qilin pill therapy for varicocele could significantly improve the semen quality after operation in patients with asthenospermia.
