甘露聚糖肽注射液预防乳腺手术切口感染62例的临床观察

摘　　要：目的：探讨单用甘露聚糖肽注射液预防乳腺手术切口感染的临床效果及可行性.方法：将124例乳腺手术患者随机分为试验组和对照组.试验组患者术前1小时、术后1～6天应用甘露聚糖肽注射液,静脉点滴,10mg/天,但不给予任何抗生素.对照组患者术前1小时、术后1～6天静脉点滴相同剂量的生理盐水,且不给予任何抗生素.观察记录患者临床体征、实验室检查及免疫学指标.结果：术前两组免疫学指标无统计学差异,术后对照组CD3＋、CD4＋、CD4＋/CD8＋、NK细胞、C3、C4明显下降,但试验组术后各项免疫学指标较术前、术毕明显升高,且与对照组比较具有统计学差异（P〈0.05）.试验组临床总有效率略高于对照组,术后7天更为明显,但两组比较无统计学意义（P=0.61,P=0.26）.试验组患者平均住院时间较对照组显著缩短（P=0.037）,且感染并发症发生率也明显降低.结论：甘露聚糖肽注射液能提高乳腺患者机体免疫功能,在不使用抗生素的情况下,能有效预防手术创口感染,且安全性好.Objective： To observe the clinical effect and feasibility of mannatide injection in preventing breast operation incision infection singlely.Methods：124 patients were divided into treatment group（n=62）and control group（n=62） randomly, The patients in treatment group were administrated mannatide injection from one hour before operation to 1weeks post-operation,and the use of antibiotics were forbidden.The control group was only given the same dose of normal saline.The clinical symptoms,laboratory tests immunological indices were observed and recorded.Results：There were no significant differences in the content of immunity index between the two groups before operation. The content of CD3＋,CD4＋,CD4＋/CD8＋,NK,C3 and C4was decreased significantly post-operation.All of the immunity indexes were superior to preoperation in treatment group significantly at the forth postoperative day and the seventh postoperative day. here were significant differences between two groups（P 〈 0.05）at post-operation. The total infection of the treatment group was higher than contral group, but there were no significant differences between two groups（P=0.61,P=0.26）.In the treatment group the average hospitalization time is significantly shorter than those in control group （P=0.037 ）, and the incidence of infectious complications was also significantly decreased.Conclution：Mannatide injection can improve the immune function of patients with breast without the use of antibiotics, prevent wound infection effectively and safely.
