大黄联合活性炭防治创伤性休克患者胃肠功能障碍的临床疗效观察

摘　　要：目的探讨大黄联合活性炭防治创伤性休克患者胃肠功能障碍的临床效果。方法选取2013年9月-2014年9月我院收治的60例急诊或在院诊断创伤性休克患者,将患者随机分为大黄联合活性炭治疗组和常规防治组。常规防治组给予常规防治,大黄联合活性炭组在常规防治组的基础上给予大黄联合活性炭防治。再选取同期30例体检者为正常对照组。治疗72 h后,比较胃泌素、血清肠型脂肪酸结合蛋白(I-FABP)、二胺氧化酶(DAO)水平的变化。治疗前后患者的肛门排气排便、肠鸣音、腹胀情况。结果治疗后大黄联合活性炭组的胃泌素、I-FABP、DAO水平接近正常对照组,低于常规防治组,差异具有统计学意义(P<0.05)。大黄联合活性炭组排气排便、有肠鸣音的发生率高于常规防治组,腹胀发生率低于常规防治组,差异具有统计学意义(P<0.05)。结论大黄联合活性炭可降低胃泌素、I-FABP、DAO水平,促进创伤性休克患者的胃肠道蠕动。[ Objective ] To investigate the clinical effect of rhubarb combined with activated carbon in prevention and treatment of traumatic shock patients with gastrointestinal dysfunction. [ Methods ] 60 cases of emergency in our hospital or in the hospital diagnosis of patients with traumatic shock, the patients were randomly divided into the rhubarb combined with activated carbon treatment group and routine treatment group. Normal control group was given conventional treatment, rhubarb combined with activated carbon group received rhubarb combined with activated carbon control based on conventional control group on. Then select the same period in 30 healthy persons as normal control group. After 72 h treatment, comparison of serum gastrin, I-FABP, DAO levels of change. Before and after treatment in patients with anal flatus and defecation, abdominal distension, bowel cries sound situation. [ Results ] Rhubarb gastrin activated carbon group, I-FABP, DAO levels close to normal control group after treatment, lower than the conventional treatment group, the difference was statistically significant（P〈0.05）. Rhubarb charcoal group exhaust defecation, there is a high incidence of bowel sounds in the conventional treatment group, the incidence of bloating lower than conventional treatment group, the difference was statistically significant（P〈0.05）. [ Conclusion ] Rhubarb activated carbon can be reduced gastrin, I-FABP, DAO levels in patients with traumatic shock promote gastrointestinal motility.
