新疆某二级医院实施临床路径的3个病种住院日及住院费用分析

摘　　要：目的分析2012年1月至2013年12月新疆某二级甲等医院临床路径与非临床路径患者的住院日及各项住院费用,为广泛开展临床路径提供研究基础。方法采用回顾调查的方法,抽取本院阑尾炎153例、剖宫产700例、腰椎间盘突出症277例的临床路径与非临床路径住院患者的住院病历,采用t检验及t′检验对比分析住院天数、术前、术后天数、各项住院费用等测量指标。结果临床路径与非临床路径患者比较,进入临床路径患者总住院天数、手术前后住院天数、总住院费用、药费、材料费、检查费均显著减少,差异具有统计学意义。其中腰椎间盘突出症患者中临床路径组住院日平均下降3.95天,材料费降低了25.2%;阑尾炎患者中临床路径组药费降低了21.8%。结论临床路径的实施有助于减少无效、低效住院日,控制住院天数,严格执行药物管理、减少检查费,降低住院费用。Objective To provide study basis for clinical pathway, hospitalization days and cost of patients in clinical/non clinical path were analyzed in a general hospital of level II in Xinjiang from Jan. 2012 to Dec. 2013. Methods Hospitalization records of 153 cases with appendicitis, 277 cases with cesarean section, and 700 cases of disc herniation in clinical/non clinical path were collected from the hospital by retrospective survey method. T test and t test were used to analyze the actual days in hospital, the days of preoperative and postoperative and the hospitalization expenses. Results The data of clinical pathway group and none clinical pathway group showed that the clinical pathway group has a lower level of the actual days in hospital, the days of preoperative and postoperative, the totally costs, drug costs, material fee, examination fee. There was a significant difference between two groups. Hospitalized days were decreased an average of 3.95 days, and the cost of material was reduced by 25.2% in clinical path group of lumbar disc herniation. The expenses of medicine were decreased by 21.8% in clinical pathway group of appendicitis patients. Conclusion Implementation of clinica1 pathways can shorten the ineffective actual days in hospital, control the actual days in hospital, strict enforcement of drug management, reduce the examination fee, reduce the medical costs.
