甲氨蝶呤两种应用方案联合米非司酮及中药口服治疗异位妊娠的临床研究

摘　　要：目的：探讨甲氨蝶呤两种应用方法联合米非司酮及中药治疗异位妊娠的临床疗效和不良反应，指导临床用药。方法将60例异位妊娠患者随机分为A、B两组各30例。A组应用甲氨喋呤1mg／（kg．d），单日肌肉注射1次共4天，双日肌注四氢叶酸钙0．1mg／（kg．d）1次共4天；B组应用甲氨喋呤50mg／m2单次肌内注射，联合米非司酮50mg，口服2次／d，用3天，同时口服中药治疗。观察两组的临床疗效和不良反应。结果：B组住院天数明显少于A组，A组Ⅱ度、Ⅲ度胃肠道反应及血细胞下降发生率明显高于B组，两组比较有统计学意义（P〈0．05）。结论：使用甲氨喋呤联合米非司酮，同时口服中药治疗异位妊娠，临床疗效显著，胃肠道反应小，值得临床推广应用。Objective： To evaluate the clinical efficacy and adverse reactions of methotrexate combined with mifepristone and traditional Chinese medicine in treatment of ectopic pregnancy. Methods： 60 cases of ectopic pregnancy were randomly divided into Group A and Group B with 30 cases each. Group A were given one methotrexate lmg/（kg.d） intramuscular injection on odd days for 4 times and given one intramuscular injection of leucovorin 0.1mg/（kg.d） on even days for 4 times. Group B were given one intramuscular injection ofmethotrexate 50mg/m2 per day for 3 days, and mifepristone 50mg, orally 2 times per day for 3 days. Besides, Group B was also given traditional Chinese oral medicine. The clinical efficacy and adverse reactions were observed and compared between the two groups. Results： Hospital stay length of Group B was significantly less than that of Group A. The incidence of Grade II and Grade III gastrointestinal reactions and decrease of blood cells of Group A was significantly higher than that of Group B. The differences were statistically significant （P〈0.05）. Conclusion： Methotrexate combined with oral application of mifepristone and traditional Chinese medicine has significant clinical effect and less adverse reactions in treatment of ectopic pregnancy and is worthy of clinical application.
