肩难产的预测、预防及产程中的助产分析

摘　　要：目的探讨肩难产预测、预防以及产程中有效助产措施，为提高产妇分娩成功率及新生儿生活质量提供可靠依据。方法对滨州市中心医院2011年1月1日至2012年12月31日进行分娩并发生肩难产的21例患者的临床资料进行回顾性分析，包括患者分娩前各项资料，分娩过程中临床表现以及处理方式，总结肩难产预测及预防方法、产程中有效助产措施等。结果21例发生肩难产产妇均顺利分娩，未出现产妇或新生儿死亡现象，其中2例新生儿出现锁骨骨折，1例新生儿出现臂丛神经损伤，经对症治疗预后良好；1例产妇出现软产道裂伤，经对症治疗后痊愈。结论对围生期产妇身体各项临床表现进行分析，若其符合肩难产预测条件，可进行及时有效的预防措施，防止产妇在分娩过程中出现肩难产并发症；进行必要的产前准备，若患者分娩过程中出现肩难产可及时给予正确助产措施，提高分娩成功率，保障产妇及新生儿生活质量与生命安全。Objective To investigate the prediction, prevention and effective midwifery of shoul- der dystoeia, and to provide reliable basis for improving the success rate of maternity and neonatal life quality. Methods The clinical data of 21 patients with shoulder dystocia in Binzhou central hospital from January 1st, 2011 to December 31th, 2012 were retrospectively analyzed, including data of patients be- fore delivery of various, clinical manifestations and treatment in the process of childbirth, and methods of forecasting and preventing shoulder dystocia and the effective measures of midwifery in labor. Results Twenty-one cases of shoulder had delivery successfully, and there was no maternal woman or neonatal death occurred. Two cases of neonatal had clavicular fracture, and 1 case had neonatal brachial plexus in- jury, and they had good prognosis after symptomatic treatment. One case appeared maternal soft birth ca- nal laceration and recovered after symptomatic treatment. Conclusions The clinical manifestations of perinatal maternal body should be analyzed, and if it meet the requirements for the shoulder dystocia, we can use timely and effective prevention measures to prevent the occurrence of shoulder dystocia complica- tions during childbirth. If there is necessary antenatal preparation, we can give right midwife measures timely when shoulder dystocia appeared in the process of childbirth to improve the success rate of delivery and ensure the safety of maternal women and neonatal life quality and life safety.
