摘 要:目的:寻求血清胆碱酯酶和血乳酸在小儿危重症病情评估中的应用价值。方法:选择我院儿科住院患儿225例,分为非危重组80例,危重组80例,极危重组65例,入院后即采动脉血检测血乳酸浓度,并于24h内检测血清胆碱酯酶活性,分析血清胆碱酯酶、血乳酸与PCIS评分之间的关系。结果:极危重组的血清胆碱酯酶明显低于危重组,危重组明显低于非危重组,而极危重组的血乳酸明显高于危重组,危重组明显高于非危重组,差异均有统计学意义(P<0.05),患儿PCIS评分越低,病情越重,血清胆碱酯酶越低,两者呈正相关,而PCIS评分越低,血乳酸越高,两者呈负相关。结论:危重儿的血清胆碱酯酶和血乳酸对小儿危重症的病情评估具有一定预警价值。Objective: To find out the pediatric critical illness. Methods: app 225 lication value of serum cholinesterase and blood lactic acid in children admitted into our hospital were divided into the non-critical group (80 cases), critical group (80 cases) and extremely critical group (65 cases). Once admitted into the hospital, arterial blood samples were collected for the concentration detec tion of blood lactic acid, and the serum cholinesterase activity was detected within 24h. Then the relationships between cholinesterase, blood lactic acid and PCIS were analyzed. Results: The serum cholinesterase of the extremely critical group was significantly lower than that of the critical group, and critical group was lower than the non-critical group; and the blood lactic acid concentration of the extremely critical group was obviously higher than that of the critical group, and the critical group was higher than the non-critical group, which were of significantly differences (P〈0.05). The lower of the PClS, the more critical of the illness, and the lower of the serum cholinesterase, which were positively correlated. While the lower of the PCIS, the higher of the blood lactic acid concentration, which was negatively correlated. Conclusion: The serum cholinesterase and blood lactic acid have the early warning value for the evaluation of pediatric critical illness.
