摘 要:目的探讨抗环瓜氨酸肽抗体(抗CCP)定量检测在类风湿性关节炎(RA)中的应用价值。方法临床确诊类风湿性关节炎患者150例(RA组),其他自身免疫性疾病患者135例(非RA组),健康体检者120例(对照组),对其进行抗CCP、类风湿因子(RF)、超敏C反应蛋白(hs-CRP)的定量检测,比较其相关性。结果 RA组抗CCP、RF含量明显高于非RA组和对照组,差异有统计学意义(P<0.01),hs-CRP含量与非RA组差异无统计学意义(P>0.05),但均与对照组差异有统计学意义(P<0.01)。非RA组抗CCP、RF含量与对照组差异无统计学意义(P>0.05)。RA组抗CCP、RF、hs-CRP含量明显与RA患者关节受损程度呈正相关(r分别为0.741、0.654、0.432,P<0.01)。以抗CCP>40 U/ml、RF>20 U/ml为阳性标准,比较其对RA患者诊断的敏感度和特异度,抗CCP敏感度略低而特异度较高。结论抗CCP定量检测因其操作简单、特异性较高,对RA的临床诊断具有较大价值。Objective To discuss the application value of ring citrulline peptide antibody(CCP) resistance quantitative detec- tion in the diagnosis of rheumatoid arthritis(RA). Methods The clinical diagnosed 150 cases of patients with rheumatoid ar- thritis( RA group), and 135 cases of patients with other autoimmune diseases( non- RA group) and 120 cases of healthy con- trois ( control group) were selected, uneffection anti - CCP, rheumatoid factor ( RF), hypersensitive c - reactive protein ( hs - CRP) quantitative detection were performed, and the correlation among them was compared. Results Anti - CCP, RF content in RA group was obviously higher than that of the non - RA group and the control group, and the difference had statistical signif- icance(P 〈0.01 ), and the difference on the hs -CRP levels between RA group and the non -RA group was not statistically significant ( P 〉 0. 05 ), but the differences of both of them were all statistically significant compared to the healthy control group ( P 〈 0.01 ). The difference of anti - CCP and RF content between the non - RA group and the control group was not statistically significant( P 〉0.05 ). The CCP, RF, and hs - CRP content of RA group were significantly positively correlated (r was 0. 741, 0. 654 and 0. 432, P 〈0. 01 ). Regarding anti- CCP 〉 40 U/m1, RF 〉 20 U/ml as positive standard, to compare the sensitivity and specificity of diagnosis of RA patients, anti - CCP had slightly lower sensitivity and higher specificity. Conclu- sion Anti - CCP quantitative detection, because of its simple operation and higher specificity, has a larger value in the clinical diagnosis of RA.
