摘  要:目的探讨人工耳蜗植入术中电诱发镫骨肌反射(elcetrically evoked stapedius responses,ESR)及神经反应遥测(neural response telemetry,NRT)的应用及临床意义。方法对22例行Nucleus 24 Contour型人工耳蜗植入者,在术中应用Cochlear公司提供的NRT 3.0软件分别进行术中ESR和NRT检测,选取蜗底(1、3号电极)、蜗中(11、13号电极)及蜗顶(20、22号电极),分别检测对应的ESR阈值及NRT阈值。结果 ESR和NRT检出率分别为96.21%(127/132)和95.45%(126/132),ESR及NRT阈值由蜗底至蜗顶呈逐渐下降的趋势(P<0.05),ESR与NRT阈值之间呈正相关(r=0.53,P<0.05)。结论人工耳蜗植入术中应用ESR及NRT可以及时确认植入体植入情况,并且为术后调机提供客观的参考。Objective To investigate the application and clinical significance of electrically evoked stapedius reflex (EESR) and neural response telemetry (NRT) during cochlear prosthesis implantation. Methods A total of 22 patients underwent Nucleus 24 Contour type of cochlear prosthesis implantation, and the values of intraoperative EESR and NRT were detected using NRT 3.0 software provided by Cochlear corporation. The corresponding liminal values of EESR and NRT were respectively detected for basis cochleae ( number 1 and 3 electrodes), within concha labyrinthi ( number 11 and 13 electrodes) and cupula cochleae (number 20 and 22 electrodes). Results The detection rates of EESR and NRT were 96.21% (127/132) and 95.45% (126/132) , and the liminal values of EESR and NRT gradually decreased from the values of basis cochleae to cupula cochleae ( P 〈 0. 05 ), and the EESR liminal values were positively correlated with NRT liminal values (r = 0. 53, P 〈 0. 05 ). Conclusion The electrically evoked stapedius reflex and neural response telemetry in application of cochlear prosthesis implantation can quickly confirm the condition of implant button, and help cochlear programming after the operation.
