三伏贴联合西药治疗妊娠期支气管哮喘慢性持续期50例临床观察  

Dog-days Paste Combined with Western Medicine in Treatment of Pregnancies with Bronchial Asthma during Chronic and Continuous Period: A Fifty Cases Observation

摘　　要：目的观察三伏贴治疗妊娠期支气管哮喘(妊娠哮喘)慢性持续期患者的临床疗效及可能作用机制。方法将100例妊娠哮喘慢性持续期患者随机分为对照组和治疗组各50例。对照组给予布地奈德气雾剂吸入治疗,每次256μg,每天1次,共30天。治疗组在对照组的基础上给予三伏贴治疗,于头伏、二伏、三伏的第1天各治疗1次,末次贴敷10天后观察两组患者临床疗效、症状和体征缓解时间,检测两组患者治疗前后肺功能、血清降钙素(PCT)、25-羟维生素D3、血清C反应蛋白(CRP)水平及唾液中免疫球蛋白A(SIg A)、褪黑素和皮质醇水平。结果治疗后两组患者血清PCT、CRP水平较治疗前降低,25-羟维生素D3、SIg A、褪黑素和皮质醇水平提高,肺功能改善,差异均有统计学意义(P<0.05)。治疗组治疗后血清PCT和CRP较对照组明显降低,25-羟维生素D3、SIg A、褪黑素和皮质醇水平、肺功能明显提高(P<0.05)。治疗组喘息、咳嗽、肺部哮鸣音、胸膈满闷缓解时间明显短于对照组(P<0.05)。治疗组临床疗效总有效率91.49%,对照组为75.00%,治疗组疗效优于对照组(P<0.05)。结论三伏贴可能通过降低血清PCT、CRP水平,提高血清25-羟维生素D3和唾液中SIg A、褪黑素和皮质醇水平使妊娠哮喘慢性持续期患者临床症状得到有效控制。Objective To observe clinical curative effect and possible mechanism of Dog-days Paste treated pregnancies with chronic and continuous bronchial asthma（ pregnancy asthma）. Methods One hundred pregnancies with chronic and continuous asthma were randomly divided into control group and treatment group,with 50 cases in each group. The control group was given budesonide aerosol inhalation therapy,256 μg each time,once a day,a total of 30 days. The treatment group was given Dog-days Paste therapy on the basis of the control group,once a day that the first day in each period of the hot season. After the last applied 10 days,observed clinical efficacy,signs and symptoms remission time of both groups and detected pulmonary function,serum calcitonin（ PCT）,25-hydroxyl vitamin D3[25-（ OH） D3],serum c-reactive protein（ CRP） levels and saliva immunoglobulin A（ SIg A）,melatonin and cortisol levels before and after treatment. Results After treatment,serum PCT and CRP levels decreased,25-（ OH） D3,SIg A,melatonin and cortisol levels increased and pulmonary function improved in both groups. The difference was statistically significant（ P〈0. 05）. After treatment,serum PCT and CRP of the treatment group were significantly lower than the control group,25-（ OH） D3,SIg A,melatonin and cortisol levels and pulmonary function improved significantly（ P〈0. 05）. Remission time of wheezing,cough,pulmonary wheezing and choking sensation in chest and diaphragm in the treatment group,were significantly shorter than those in the control group（ P〈0. 05）. Total effective rate in the treatment group was 91. 49% and that of the control group was 75. 00%. The treatment group was better than the control group（ P〈0. 05）. Conclusion Dog-days Paste may effectively control clinical symptoms of pregnancies with chronic and continuous asthma by lowering serum PCT and CRP levels; also increasing the serum 25-（ OH） D3; saliva SIg A; melatonin and cortisol levels.
