抵当汤及其拆方对糖尿病大鼠心肌病变的影响

摘　　要：目的观察抵当汤及其拆方对糖尿病大鼠心肌病变的疗效。方法 105只SD大鼠采用单次腹腔注射链脲佐菌素55 mg/kg溶液制备糖尿病大鼠模型,将造模成功的100只大鼠随机分为模型组、抵当汤组、桃仁大黄组、水蛭虻虫组和缬沙坦组,每组20只;另以10只正常SD大鼠作为对照组。抵当汤组、桃仁大黄组和水蛭虻虫组灌胃相应中药10 g/(kg·d),缬沙坦组灌胃缬沙坦胶囊80 mg/(kg·d),对照组和模型组灌胃等量蒸馏水,每天1次。8周后光镜下观察各组大鼠左心室心肌组织HE染色情况;Masson染色观察心肌纤维化改变,并计算心肌组织胶原面积占总面积的比值;电镜观察心肌细胞超微结构变化。结果 HE染色结果显示,模型组大鼠左心室心肌细胞肥大,排列紊乱,并有炎症细胞浸润;抵当汤组和缬沙坦组心肌细胞排列较整齐,细胞间隙减小,间质和血管周围细胞外基质减少。与对照组比较,模型组心肌组织胶原面积/总面积升高(P<0.01);抵当汤组胶原面积/总面积较模型组显著降低(P<0.01),而桃仁大黄组和水蛭虻虫组胶原面积/总面积无明显变化(P>0.05)。电镜心肌细胞超微结构结果显示,模型组肌节显示模糊不清,肌原纤维溶解,甚至消失;抵当汤组和缬沙坦组肌节显示整齐,线粒体形态呈椭圆形,结构大致正常。结论抵当汤能改善糖尿病大鼠心肌纤维化的病理程度,对糖尿病心肌病具有防治作用。Objective To observe the curative effect of Didang Decoction and its disassembled formula on myocardial lesion in diabetic rats. Methods Totally 105 SD rats were injected streptozotocin solution intraperitoneally with 55 mg / kg to prepare the diabetic rat model,100 rats of successful model were randomly divided into model group,Didang Decoction group,Taoren（ Semen Pruni persicae） Dahuang（ Radix Rumici Japonici） group,Shuizhi（ Leech） Mangchong（ Tabanus） group and valsartan group,with 20 in each group. The other 10 normal SD rats served as the control group. Didang Decoction group,Shuizhi Mangchong group and Taoren Dahuang group gavage corresponding Chinese medicine 10 g /（ kg·d）,valsartan group gavage valsartan capsule 80 mg /（ kg·d）,the control group and the model group were perfused with distilled water,1 time a day. After 8 weeks,HE staining in left ventricular myocardium tissue of the rats in each group would be observed under light microscope; the changes of myocardial fibrosis are observed by Masson staining,and the collagen area of myocardium is calculated; ultrastructure changes of myocardial cells are observed by electron microscope. Results The results of HE staining showe that the left ventricular myocardial cells in the model group are enlarged and arranges in disorder,and there is inflammatory cells infiltration; The arrangement of myocardial cells in the Didang Decoction group and valsartan group is relatively neat,the intercellular space is decreased,interstitial and perivascular extracellular matrix is decreased. Compared with the control group,the ratio of the collagen area to the total area of myocardium in the model group is increased（ P〈0. 01）; The ratio of the collagen area to the total area in the Didang Decoction group is significantly decreased than that in the model group（ P〈0. 01）,however it is of no significant change in the Taoren Dahuang group and Shuizhi Mangchong group（ P〉0. 05）.
