三种抗精神病药物对血糖血脂及体质量的不良影响

摘　　要：目的:探讨3种抗精神病药物(齐拉西酮、利培酮、氯丙嗪)对精神分裂症患者血糖血脂及体质量的不良影响。方法:180例精神分裂症患者根据临床研究需要分为齐拉西酮组(入组60例,完成58例)、利培酮组(入组60例,完成57例)、氯丙嗪组(入组60例,完成55例),分别在治疗前及治疗后4周、8周监测空腹血糖、甘油三酯、胆固醇及体质量等各项指标,分析3组内治疗前后数据的差异,并比较3组间数据的差异。结果:氯丙嗪组及利培酮组患者的甘油三酯及胆固醇明显高于治疗前(P<0.05),齐拉西酮与治疗前相比无明显变化(P>0.05),治疗后血脂升高程度:氯丙嗪>利培酮>齐拉西酮,差异均有统计学意义(P<0.05);3组中氯丙嗪、利培酮组体质量与治疗前比较均升高(P<0.05),齐拉西酮组与治疗前相比无显著性差异(P>0.05)。3组抗精神病药物对于血糖影响与治疗前比较均无统计学意义(P>0.05)。结论:氯丙嗪和利培酮对血脂、体质量的影响大于齐拉西酮。Objective. To study the adverse effects of three antipsychotic drugs on glucose metabolism, lip- id metabolism and body weight in schizophrenic patients. Methods： One hundred and eighty schizophrenic patients were divided into ziprosidone group, risperidone group and chlorpromazine group. Before and after four-week and eight-week treatment, the body weight, fasting bloob glucose, cholesteral and triglyceride were measured. In addi- tion, the body mass index （BMI） was calculated. Results：After treatment,the blood cholesteral and triglyceride concentrations significantly increased in risreridone and chlorpromazine groups. The blood lipid in ziprosidone group had no significant changes after the treatment. The BMI increased in a greater magnitude in risreridone and chlor- promazine groups. The fasting blood glucose had no significant changes in the three groups. Conclusion： Risperi- done and chlorpromazine have more severe adverse effects on lipid metabolism and boy weight than ziprosidone.
