锁骨钩钢板治疗CraigⅡB型锁骨远端骨折钢板取出前后肩关节功能的变化

摘　　要：目的探讨锁骨钩钢板治疗CraigⅡB型锁骨远端骨折钢板取出前后肩关节功能变化。方法收录2009年1月-2014年1月我院13例锁骨远端骨折病例并随访,复查X线并进行Constant-Murley评分。结果 13例平均随访日寸间(51.77±4.79)个月。X线片显示,所有患者愈合良好,某些可见锁骨钩周围骨质溶解或肩锁关节创伤性关节炎征象。取板后的ConstantMurley评分(88.08±16.32)高于取板前(75.31±9.04)(P=0.003),患肩功能得到明显改善,取板后肩部疼痛明显减轻,活动性增强,活动度增大(P<0.05),但取板前后上肢肌力的差异无统计学意义(P=0.303)。结论骨折愈合良好的情况下尽早取出锁骨钩钢板,有助于减少疼痛、恢复肩关节功能和提高生活质量。Objective To explore the changes of functional outcome after removal of the hook platefixation of Craig type Ⅱ B distal clavicle fractures. Methods Thirteen patients who had clavicular hook plate internal fxation for distal clavicle fracture in our hospital from January 2009 to January 2014 were followed up, and their X-ray and shoulder function were evaluated by Constant- Murley score. Results All patients were followed up for （51.77 ± 4.79） months. X-rays showed osteolysis around the hook and signs of traumatic arthritis of the acromioclavicular joint. The Constant-Murley score after removal of the hook plate （88.08 ± 16.32） was significantly higher than before （75.31 ± 9.04） （P 〈 0.05）, with the features of obvious pain relief, normal functional activities and wider range of motion （P 〈 0.05）. But no significant difference was found in muscle strength before and after removal of the hook platefixation （P 〉 0.05）. Conclusion It is important to inform the patients about the necessity of removal of the clavicular hook plates in time as recommended to relieve pain, recover shoulder function and improve life quality effectively.
