云南白药联合生长抑素治疗肝硬化合并上消化道大出血临床观察

摘　　要：目的观察云南白药联合生长抑素治疗肝硬化合并上消化道大出血的临床疗效。方法将100例患者随机分为实验组和对照组各50例,对照组予以常规治疗的基础上,加用生长抑素治疗;实验组在对照组基础上,给予云南白药药粉口服,如有困难经过胃管注入。对比两组患者治疗48 h后的血清学指标、输红细胞量、止血时间、24 h止血率、再出血发生率和不良反应发生率。对比两组患者的临床疗效和转归。结果实验组治疗48 h后的血红蛋白、血小板、纤维蛋白原和红细胞压积均显著高于对照组(P<0.01)。实验组输红细胞量、止血时间和再出血发生率显著低于对照组,24 h止血率显著高于对照组(P<0.05)。两组患者不良反应发生率相当(P>0.05)。实验组治疗后的临床总有效率90.00%,明显高于对照组的68.00%(P<0.05)。两组患者的死亡率相当(P>0.05)。结论云南白药联合生长抑素治疗肝硬化合并上消化道大出血临床疗效显著,安全性高,值得临床应用。Objective： To analyze the clinical efficacy of somatostatiu combined with Yunnan Baiyao on the treatment of liver cirrhosis with gastrointestinal bleeding. Methods： 100 cases of gastrointestinal bleeding on the merger of cirrhosis in our hospital from August 2012 to August 2014 were randomly divided into experimental and control groups. The control group received somatostatin based on conventional therapy;the experimental group were injected Yunnan Baiyao powder into the stomach based on the treatment of the control group. The following data of patients in the two groups were compared ： serum markers 48 hours later, red cell volume, bleeding time, 24 hours hemostasis rate, rebleeding rate and incidence of adverse reactions. Clinical efficacy and mortality were also compared between the two groups. Results： Hemoglobin platelets ,fibrinogen and hematocrit after 48 hours of treatment in the experimental group were significantly higher （P〈 0.01 ）. In the experimental group,red cell vol- ume,hemostatic time and rebleeding rate were significantly lower than those of the control group;24 hours bleed- ing rate was significantly higher （P〈 0.05）. The incidence of adverse reactions of both groups was no significant difference （P〉0.05）. The clinical efficacy of the experimental group after treatment was significantly better than that of the control group （P〈 0.05 ）. Patients in the two groups were no significant differences in mortality （P〉 0.05 ）. Conclusions： The treatment of cirrhosis of Yunnan Baiyao somatostatin combined with gastrointestinal bleeding clinically has significant effect,safe,which is worthy of clinical application.
