摘 要:目的了解孕晚期妇女生殖道感染病原菌的分布及耐药性,为临床预防和治疗围生期母婴相关的细菌感染提供依据。方法回顾性分析我院产科2011年—2013年住院孕晚期妇女生殖道病原菌的微生物学资料。结果 9 373例孕妇中共分离到1 442株病原菌,检出率为15.4%,其中真菌1 156株,占80.1%,以白假丝酵母为优势菌,占真菌的59.6%;革兰阴性菌188株,占13.0%,以大肠埃希菌为主;革兰阳性菌98株,占6.8%。真菌对伊曲康唑的耐药性最强,其次是氟康唑,革兰阴性菌和革兰阳性菌对抗菌药物的耐药率因菌种不同而有所差异。结论孕晚期妇女生殖道感染病原菌以真菌为主,且对伊曲康唑的耐药率最高,临床治疗时应根据病原菌种类和耐药性结果合理使用抗菌药物。ObjectiveTo investigate the distribution and drug resistance of pathogens isolated from third trimester women's reproductive tract,and provide the basis for clinical treatment.MethodsMicrobiological data of pathogens in reproductive tract of third trimester women in our hospital during 2011-2013 were retrospectively analyzed. Results1442 pathogens were isolated from 9373 cases of pregnant women,which consisted of 1156 Fungi,188 Gram-negative bacteria and 98 Gram-positive bacteria,and the detection rate is 15.4%; Candida albicans and Escherichia coli accounted for the major part of the Fungi and Gram-negative bacteria respectively. Gram-positive bacteria accounted for 6.8% of the total. The results of antimicrobial susceptibility test indicated that fungi displayed the strongest resistance to itraconazole,followed by fluconazole; Gram-negative and gram-positive bacteria showed different resistance rates since they belong to different species.ConclusionFungi is the dominant in pathogens isolated from third trimester women's reproductive tract,and they showed strongest resistance to itraconazole. Clinical treatment should be based on rational use of antimicrobial drugs according to pathogens and drug resistance results.
