摘 要:目的探讨丹参注射液联合布地奈德/福莫特罗治疗急性加重期老年COPD患者的临床疗效。方法收集2011年1月—2013年12月泰安市中心医院收治的216例急性加重期老年COPD患者,随机分为2组,每组108例。对照组给予布地奈德/福莫特罗,观察组在对照组基础上给予丹参注射液。治疗前、后,检测2组T细胞亚群(CD3+、CD4+、CD+8)、细胞因子(IL-6、IL-8、IL-17A、TNF-α)水平、氧化应激指标(SOD、GSH-PX、MDA),对肺功能指标(FEV1%pred、FEV1/FVC)、SGRQ评分、6MWT进行评价。结果治疗后,2组CD3+、CD4+水平显著升高(P<0.05),观察组CD3+、CD4+水平显著高于对照组(P<0.05)。治疗后,2组IL-6、IL-8、IL-17A、TNF-α水平显著降低(P<0.05),观察组IL-6、IL-8、IL-17A、TNF-α水平显著低于对照组(P<0.05)。治疗后,2组SOD、GSHPX水平显著升高(P<0.05),观察组SOD、GSH-PX水平显著高于对照组(P<0.05);治疗后,2组MDA水平显著降低(P<0.05),观察组MDA水平显著低于对照组(P<0.05)。治疗后,2组FEV1%pred、FEV1/FVC水平显著升高(P<0.05),且观察组FEV1%pred、FEV1/FVC水平显著高于对照组(P<0.05)。治疗后,2组SGRQ评分显著降低(P<0.05),观察组SGRQ评分显著低于对照组(P<0.05)。治疗后,2组6MWT显著升高(P<0.05),观察组6MWT显著高于对照组(P<0.05)。结论丹参注射液联合布地奈德/福莫特罗可有效调节急性加重期老年COPD患者免疫功能、降低炎性和氧化应激反应,改善患者肺功能。Objective To investigate the efficacy of salvia miltiorrhiza injection combined with budesonide and salmeterol inhalation in the treatment of elder patientswith acute exacerbation of chronic obstructive pulmonary disease( COPD).Methods A total of 216 cases of elder patientswith acute exacerbation of COPD from January 2011 to December 2013 in our hospital were selected and randomly divided into two groups with108 cases in each group. The patients in the control group were given budesonide and salmeterol inhalation,and the patients in the observation group were given salvia miltiorrhiza injection based on the treatment of the control group. Before and after the treatment,T cell subsets( CD3+,CD4+,CD8+),inflammatory cytokines(IL-6,IL-8,IL-17 A,TNF-α),oxidative stress indicators(SOD,GSH-PX,MDA) of both groups were detected,andthe lung function( FEV1% pred,FEV1/ FVC),SGRQ scores,and 6MWT were evaluated. Results After the treatment,CD3+,CD4+levels of both groups were significantly increased( P 〈0. 05),and CD3+,CD4+levels of the observation group were significantly higher than those in the control group( P 〈0. 05). After the treatment,IL-6,IL-8,IL-17 A and TNF-α levels of both groups were significantly reduced( P 〈0. 05),and dramatically lower in the observation group as compared with the control group( P 〈0. 05). After the treatment,SOD and GSH-PX levels of both groups were significantly increased( P 〈0. 05),and SOD and GSH-PX levels of the observation group were significantly higher than those of the control group( P 〈0. 05);After the treatment,MDAlevels of both groups were significantly reduced( P 〈0. 05),and MDA level of the observation group was significantly low than that of the control group( P 〈0. 05). After the treatment,FEV1% pred and FEV1/ FVC levels of both groups were significantly increased( P 〈0. 05),and FEV1% pred and FEV1/ FVC levels of the observation group were significantly higher than those of the control group
