病毒性心肌炎患者血清粘附分子-1的变化及意义

摘　　要：目的对病毒性心肌炎患者血清ICAM-1水平进行检测,为临床判断病情、指导临床治疗提供新的思路与依据。方法选取2010年7月—2014年7月收治的108例病毒性心肌炎患者作为研究对象,106例正常健康体检者作为对照组,其中108例患者轻度有38例,中度37例,重度33例;在患者入院后抽静脉血3 ml,迅速分离血浆,置-80℃低温保存,采用ELISA法检测各组的粘附分子-1水平,操作和结果计算均按说明书进行。2组ICAM-1水平比较采用t检验,不同病情轻重比较采用方差分析,其中两两比较采用t检验。结果病毒性心肌炎患者的血清ICAM-1较对照组明显升高,与正常对照组比较差异有统计学意义(t=13.376,P=0.000);不同轻重程度的患者血清ICAM-1水平不全相同(F=15.3,P=0.022),差异有统计学意义;轻度组与中度组比较差异有统计学意义(P<0.05),与重度组比较差异有统计学意义(P<0.01),中度组与重度组比较差异有统计学意义(P<0.01)。结论血清ICAM-1参与了心肌炎的病理过程,可反映机体疾病的免疫损伤程度,且与病情相关,病毒性心肌炎患者的病情越重,血清ICAM-1水平越高,为临床上诊断、治疗本病提供了新的依据。Objective To explore the change of serum intercellular cell adhesion molecule-1（ ICAM-1） and its clinical significance in the patients with viral myocarditis,andprovide new ideas and evidence to evaluate the disease condition and guide clinical treatment. Methods A total of 108 cases of viral myocarditis（ study group） and 106 healthy subjects（ control group） in our hospital from July 2010 to July 2014 were enrolled into this study. The patients in the study included 38 mild cases,37 moderate cases and 33 severe cases. The samples of venous blood were prepared at admission to test the levels of ICAM-1 by using ELISA. ANOVA and t-test were used to analyze the data. Results Serum ICAM-1 level was highest in the patients with viral myocarditis（ P 〈0. 01）,serum ICAM-1 level were dramatically different among slight group,moderate group and severe group（ P 〈0. 05 to 0. 01）. Serum ICAM-1 level was positive correlation with the severity of the patients. Conclusion ICAM-1 is involved in the pathogenesis of viral myocarditis,its level can reflect the damage degree of body＇s immunity,and associated with the condition of the patients,positively correlated with the severity.
