摘  要:目的探讨细菌主动外排泵adeB基因在临床分离的泛耐药鲍曼不动杆菌耐药性中的作用。方法琼脂稀释法检测临床分离泛耐药鲍曼不动杆菌经外排泵抑制剂羰基氰氯苯腙(CCCP)处理前后对抗生素最小抑菌浓度(MIC)变化,以聚合酶链反应(PCR)、逆转录-聚合酶链反应(RT-PCR)检测外排泵基因ade B及其表达水平。结果 50株泛耐药鲍曼不动杆菌临床株加入CCCP后,MIC值小于原值的1/4或1/4以下的四环素42株(84%)、氧氟沙星45株(90%)、亚胺培南45株(90%)、头孢噻肟50株(100%)、庆大霉素50株(100%)。50株泛耐药鲍曼不动杆菌临床株和4株敏感菌株均检测到ade B基因,但泛耐药株表达水平明显高于敏感株(P<0.01)。结论泛耐药鲍曼不动杆菌耐药性与外排泵adeB基因高表达密切相关。Objective To explore the role of bacteria efflux pump adeB gene work in the drug resistance of pan-resistant Acinetobacter baumannii islated from clinical specimens. Methods Agar dilution was used to determine the minimum inhibitory concentrations (MIC) change of antimicrobials against clinical isolates pan-resistant Acinetobacter baumannii (PRABA) that before and after processed by Carbonyl Cyanide m-Chloropheny(CCCP). Polymerase chain reaction ( PCR) and reverse transcription PCR (qRT- PCR) were utilized to determined the efflux pump adeB gene and its expression level. Results After adding CCCP into 50 isolates of pan-resistant Acinetobacter baumannii, the MIC value was of antibiotics decreased by 25% or more when compared with formers were 42 strains(84%) against tetracycline, 45 strains(90%) against levofloxacin, 45 strains(90%) against imipenem, 50 strains(100%) against cefotaxime,50 strains(100%), 50 strains(100%) against gentamicin. AdeB gene was determined in 50 pan-resistant Acinetobacter baumannii clinical strains and 4 sensitive strains, but the expression level of adeB gene of pan-resistant strains were obviously higher than sensitive strains (P〈0.01). Conclusion The drug-resistance of pan-resistant A. baumannii is closely related to the overexpression of efflux pump adeB gene.
