摘 要:目的研究2型糖尿病及肥胖患者血清腹腔脂肪型丝氨酸蛋白酶抑制剂(visceral adipose tissue-derived serine proteinase inhibitor,Vaspin)水平变化,并分析其与血糖、血脂、胰岛素抵抗指数、胰岛素的相关性,从而进一步探讨vaspin与肥胖、2型糖尿病代谢变化之间的关系。方法 2型糖尿病患者85例,分为正常体重糖尿病组及肥胖糖尿病组,同期选取我院健康体检者86名,分为对照组及单纯性肥胖组,并测定所有受试者空腹血清vaspin及其它临床及实验室指标。结果 D组vaspin水平较C组略高,C组较B组明显升高,B组较A组升高(P<0.05)。女性组的vaspin水平较男性组高(P<0.05)。相关性分析提示vaspin水平与WC、BMI及HOMA-IR呈显著正相关(P<0.01),与FPG、SBP、DBP、TG、FINS呈正相关(P<0.05),男性组vaspin与DBp呈正相关,女性组与SBp呈正相关。多元逐步回归分析提示vaspin与BMI、FPG、SBp独立相关。结论 Vaspin水平与2型糖尿病及肥胖的关系密切。Objective Study on the changes of serum visceral adipose tissue -derived serine proteinase inhibitor ( Vaspin ) levels in patients with type 2 diabetes and obesity , and analyze its correlation with blood glucose, blood lipid, HOMA-IR and FINS, in order to further explore the relationship between the vaspin and the change of metabolism of type 2 diabetes and obesity .Methods Eighty-five cases of type2 diabetes mellitus patients were selected and classified into two groups including the normal weight of diabetic group and the obese diabetic group .Meanwhile ,eighty-six healthy examinees were selected as the control group in the same period and classified into two groups including the control group and single obesity group .Serum vaspin and other clinical and laboratory parameters were determined .Results The serum vaspin levels in group D were slightly higher than those in group C , the levels in group C were significantly higher than those in group B , and in group B were higher than in group A .The serum vaspin levels in the female group were higher than those in the male group (P〈0.05).The serum vaspin levels were positively correlated with BMI , WC and HOMA -IR, and positively correlated with FPG , SBP, DBP, TG and FINS.BMI, FPG and SBP were independent factors which influenced the 1evels of serum vaspin .Conclusions Serum vaspin may be correlated with type 2 diabetes mellitus and obese .
