股骨干骨折患者围手术期血液相关性指标变化的临床意义

摘　　要：目的探讨股骨干骨折患者围手术期血液指标变化及其与下肢静脉血栓的相关性。方法将该院30例股骨干患者于手术前、术后1、3、7d采集空腹静脉血,检测其凝血4项功能、血小板计数(PLT)、D-二聚体(DD)、血液流变学指标、动脉血气分析,并进行比较分析。结果患者术后1、3d凝血酶原时间(PT)、凝血酶时间(TT)及活化部分凝血活酶时间(APTT),均较术前延长,差异有统计学意义(P<0.05);纤维蛋白原(FIB)和D-D在整个围手术期呈进行性升高,术后第3天达峰值,与术前比较差异有统计学意义(P<0.01);PLT在术后呈进行性降低,与术前比较差异有统计学意义(P<0.05),且术后第1天降到最低,差异有统计学意义(P<0.01);患者术后血液流变学各项指标、动脉氧饱和度等与术前比较,差异均有统计学意义(P<0.05);术后第7天,各项指标接近术前。结论股骨干骨折患者围手术期的血液相关性指标变化,对预防和早期发现下肢深静脉血栓有一定的临床意义。Objective To bservate the blood correlation index of patients with femoral fractures in perioperativeMethods 30 cases with femoral fractures before preoperative and postoperative 1,3,7day,all items of blood coagulation,platelet count（PLT）,D-dimer（DD）,the hemorrheology indicators,the arterial blood gas analysis indicators were detected,and compared with preoperative.Results All patients postoperative 1,3days of the prothrombin time（PT）,thrombin time（TT）and activated partial thrombo-plastin time（APTT）compared with preoperative are extended,the difference was significant（P〈0.05）;fibrinogen and DD throughout the perioperative progressive rise,and achieve the highest rise in postoperative 3day,the difference was statistically significant（P〈0.01）;PLT was progressive decrease after surgery,the difference was significant（P〈0.05）,the lowest was in postoperative 1day,the difference was statistically significant（P〈0.01）,the hemorrheology indicators,the arterial oxygen saturation in postoperative compare with the preoperative,the difference was statistically significant（P〈0.05）,postoperative in 7day,all indexs were closed to preoperative.Conclusion The changes of blood correlation index to femoral fractures patients in perioperative was existed.
