管状胃代食管对食管癌患者术后生活质量影响的研究

摘　　要：目的探讨管状胃与全胃代食管两种上消化道重建方式对食管癌切除术后患者生活质量状况影响。方法 70例食管癌患者随机分为管状胃代食管组和全胃代食管组,分别比较两组患者手术后短期并发症及术后1、6、12个月时的生活质量。结果两组患者手术并发症、围手术期死亡率无显著性差异。管状胃组术后反流性食管炎及胸胃综合征等并发症显著少于全胃组。术后1个月两组患者生活质量均较低,术后1个月两组患者QOL评分及满意度无明显差异;管状胃组术后6、12个月QOL评分及满意度均优于全胃组。结论管状胃代食管行食管癌切除术,其术后近期并发症及术后死亡率与全胃代食管手术无明确差异,但明显降低了术后并发症发生率。Objective To investigate the effect of gastric tube and whole stomach reconstruction on the life quality of patients with esophageal carcinoma after esophageal resection.Methods 70 patients with esophageal carcinoma underwent esophagectomy were enrolled and divided into gastric tube（GT）group and whole stomach（WS）group.The perioperative complications and the quality of life at 1st,6th and 12 th month after surgery in the two groups were respectively evaluated.Results There was no significant difference in the complications and the mortality during the perioperative period in the two groups.The incidence of reflux esophagitis and the thoracic stomachsyndrome in the GT group were less than in the WS group significantly.The life quality of patients in both groups was poor in 1st month postoperative,and there was no significant differences in the QOL score and the satisfacation degree in the two groups.The QOL score and the satisfaction degree of GT group in 6months and 12 months postoperative were better than that of the WS group.Conclusion There were no significant differences in the mortality rate and complication rate during the perioperative period in GT group and WS group,But the probabilities of postoperative complications were significantly reduced.
