不同鼻腔填塞材料的止血效果及对鼻黏膜损伤作用的比较

摘　　要：目的探讨与比较不同鼻腔填塞材料的止血效果及对鼻黏膜损伤的作用。方法 80只新西兰白兔随机分为碘仿组、凡士林组、止血海绵组和气囊组各20只,均行颌鼻甲部分摘除术,术后分别给予填塞碘仿纱条、凡士林纱条、止血海绵和气囊等。术后第3 d观察各组止血效果,并观察鼻黏膜损伤情况。结果止血海绵组和气囊组的止血效果明显好于碘仿组和凡士林组(P<0.05)。4组术区鼻黏膜均轻度肿胀,光镜下可见黏膜组织炎细胞浸润,扫描电镜下可见结缔组织增生,4组组间对比无显著差异;止血海绵组和气囊组的术区鼻黏膜炎细胞数明显低于碘仿组和凡士林组(P<0.05),纤毛数量在4组间无统计学差异(P>0.05)。结论相对于凡士林纱条和碘仿纱条,止血海绵和气囊行鼻腔填塞后止血效果良好,能有效抑制炎症反应,值得临床推广使用。Objective To explore and compare the hemostatic effects of different nasal cavity choking materials and their damage for nasal mucosa. Total 80 New Zealand white rabbits were divided randomly into iodoform group, Vaseline group, styptic sponge group and styptic airbag group （n = 20 per group）, on which partial removal operation of maxillary nosepieces was performed and iodoform slivers, Vaseline slivers, styptic sponges and airbags were used, respectively, after the operation. On the 3rd day after the operation, the hemostatic effects and damage for nasal mucosal in the four groups were observed. The hemostatic effects in styptic sponge group and airbag group were obviously better than those in iodoform group and Vaseline group （P〈0.05）. In all of the four groups, nasal mucosal in the operating area slightly swelled. Under light microscope, inflammatory cell infiltration was observed in the mucosal tissues and under scanning electron microscope, hyperplasia was found in the connective tissues. The inflammatory cells in the operating area in styptic sponge group and airbag group were obviously less than those in iodoform group and Vaseline group （ P〉 0.05）. The quantities of cilium in the four groups indicated no statistic difference （ P〈 0.05）. Compared with Vaseline and idoform, styptic sponge and airbag have better hemostatic effects on choking nasal cavity and restraining inflammatory reaction, so they are worthy of wider clinical application.
