216例高胆红素患儿新生儿溶血病的血清学分析

摘　　要：目的对早期出现黄疸症状的患儿,进行血清胆红素检测及新生儿ABO溶血病(ABO-HDN)溶血三项试验,并分析结果,为临床提供诊断依据。方法对216例新生儿黄疸患儿采用盐水法检测ABO、Rh血型,并检测血清胆红素浓度,对胆红素增高的患儿和母亲血型为O、婴儿为A/B型者,采用直接抗人球蛋白试验(DAT)、游离抗体试验、抗体释放试验检测,分析其构成比,统计不同母婴血型组合与HDN的关系,并观察ABO-HDN与检测日龄的关系。结果 216例新生儿胆红素浓度增高患儿样本中,确诊患者89例均为ABO血型系统不合所致,阳性率为41.20%,ABO-HDN与新生儿的A或B型的型别差异无统计学意义(P>0.05)。检测日龄与HDN的阳性率比值差异有统计学意义(P<0.05)。结论血型血清学检测对新生儿溶血病的诊断具有重要的临床价值;早期对母子ABO血型不合黄疸患儿进行溶血三项及其血清胆红素水平检测,为临床ABO-HDN的早期诊断和及时治疗提供依据。Objective Detections of serum bilirubin and ABO hemolytic disease of the newborn（ABO-HDN）were performed among children with early jaundice signs with three trials for hemolysis to provide evidence for clinical diagnosis.Methods Two hundreds and sixteen cases of neonatal jaundice were tested by saline method for ABO,Rh blood group,and serum bilirubin concentration of bilirubin.Children with high bilirubin and the mothers with blood group O and the baby with type A/B were subjected to hemolytic three tests（direct antiglobulin test,DAT）,circulating antibody test,and antibody release test）.The constituent ratio was analyzed to explore the association of maternal and child blood group with hemolytic disease of the newborn（HDN）,and ABO blood group with HDN.Results Among 216 samples of HDN,89 cases were definitely diagnosed in association with ABO blood-group incomplatibility,with a positive rate of 41.2%.HDN in ABO blood group did not show any statistical difference with A or B blood group type.Conclusion Serological detection combined with blood group test has a significant role in HDN diagnosis.The＂three tests＂for HDN and sera bilirubin axamination in early maternal-child blood group incompability may improve early diagnosis of ABO-HDN.
