耐碳青霉烯类药物鲍曼不动杆菌的碳青霉烯酶、整合酶基因型研究

摘　　要：目的研究碳青霉烯类耐药的鲍曼不动杆菌碳青霉烯酶、整合酶基因型情况。方法收集93株碳青霉烯类耐药的鲍曼不动杆菌和16株碳青霉烯类敏感的鲍曼不动杆菌,采用改良Hodge实验检测碳青霉烯酶,EDTA协同试验检测金属β-内酰胺酶;采用聚合酶链反应(PCR)扩增和测序分析碳青霉烯酶和整合子基因。结果耐药株碳青霉烯酶表型检测阳性率为62.4%,金属酶表型检测均为阴性;PCR结果显示耐药菌中OXA-23、Int1检出率为100%,OXA-51检出率为96.8%,OXA-58检出率为1.1%,未检出OXA-24、VIM、IMP和Int2,敏感株除Int1检出率为37.5%外,其余基因均未检出。结论临床分离的碳青霉烯类耐药的鲍曼不动杆菌主要携带OXA-23、OXA-51碳青霉烯酶基因型,并且均携带Ⅰ类整合酶基因。Objective To investigate the genotypes of carbapenemase and integrase by carbapenem-resistant Acinetobacter baumannii（ CRAB）. Methods A total of 93 isolates of CRAB and 16 isolates of carbapenem-sensitive Acinetobacter baumannii（ CSAB） were collected. Carbapenemases were detected by modified Hodge testing. Metallobeta-lactamase was detected by EDTA collaborative testing. Carbapenemase and integrase genotypes were amplified by polymerase chain reaction（ PCR） and sequencing. Results The carbapenemase phenotype test results showed that62. 4% isolates were positive,and metal enzymes were all negative. The PCR results for CRAB showed the detection rates of OXA-23 and Int1 were 100%,for OXA-51 was 96. 8%,for OXA-58 was 1. 1%,and OXA-24,VIM,IMP and Int2 were not found. The PCR results for CSAB showed the detection rate of Int1 was 37. 5%. Conclusions CRAB mainly carrys OXA-23 and OXA-51 carbapenemase genes,and all isolates carry ClassⅠ integrase gene.
