首次IVF-ET前行宫腔镜检查的临床意义

摘　　要：目的探讨宫腔镜检查在首次行IVF-ET患者中应用的临床价值。方法选取经阴道B超检查宫腔未见异常的首次IVF-ET患者200例,按IVF-ET治疗前宫腔镜检查情况分成三组,A组69例(宫腔镜检查正常组),B组49例(宫腔镜检查异常组),C组82例(未行宫腔镜检查组)。其中B组行相应治疗后再行IVF-ET术,A组及C组均如期行IVF-ET术。比较三组患者的临床妊娠率。结果 B组49例宫腔形态异常,包括子宫内膜炎26例,子宫内膜息肉12例,子宫内膜息肉样增生9例,子宫内膜单纯性增生过长1例,子宫黏膜下小肌瘤l例。三组IVF-ET临床妊娠率分别为A组52.2%,B组49.0%,C组26.8%;A、B两组与C组间比较差异均具有统计学意义(P<0.05),A组与B组间相比差异不显著(P>0.05)。结论宫腔镜检查虽是有创性检查,但可发现超声所遗漏的宫腔微小病变,经治疗后可显著提高IVF-ET术临床妊娠率,值得临床推广。Objective To investigate the clinical value of hysteroscopy in application of patients with first IVF-ET. Methods 200 cases of first IVF-ET patients who were detected no abnormalities in uterine cavity through transvaginal B ultrasound. All patients were divided into three groups according to the results of hysteroscopy before IVF-ET treatment： group A（69 patients with normal uterine cavity）,group B（49 patients with abnormal uterine cavity）, group C（82 patients who didn＇t implement hysteroscopy）. Group B was given corresponding treatment before IVF-ET, group A and group C were given IVF-ET on schedule. The clinical pregnancy rate of three groups was compared. Results The uterine cavity of 49 patients in group B were abnormal, including endometritis in 26 cases, endometrial polyp in12 cases, endometrial polypoid hyperplasia in 9 cases, endometrial hyperplasia in one case and uterine submucous fibroid in one case. The clinical pregnancy rate of group A, group B and group C were 52.2%, 49.0% and 26.8% respectively, the difference between group A and group C, group B and group C was statistical（P〈0.05）, the difference between group A and group B was not statistical（P〈0.05）.Conclusions Although the hysteroscopy is invasive, but it can discover minute uterine cavity lesions overlooked in ultrasonography, and can obviously improved the clinical pregnancy rate after treatment, which is worthy of promotion.
