摘  要:目的探讨剖宫产产妇腰硬联合麻醉后寒颤应用布托啡诺与曲马多治疗的临床疗效。方法根据治疗方法的不同将腰硬联合麻醉后发生寒颤的剖宫产产妇76例分为布托菲诺组和曲马多组各38例。观察两组患者的临床疗效,对比用药前和用药后5 min、15 min时Sp O2及MAP的变化以及不良反应发生情况。结果布托菲诺组治疗总有效率为94.74%,高于曲马多组的84.21%,但差异无统计学意义(P>0.05)。两组用药前后MAP比较差异均无统计学意义(P>0.05),布托诺菲组用药后5 min、15 min Sp O2与用药前比较差异显著(P<0.05)。布托诺菲组不良反应发生率(2.63%)显著低于曲马多组(31.58%),差异具有统计学意义(P<0.05)。结论剖宫产产妇腰硬联合麻醉后寒颤应用布托啡诺效果优于曲马多,且不良反应少,具有重要临床价值。Objective To explore the effect of butorphanol and tramadol on chills after combined spinal epidural anesthesia(CSEA)of parturients with cesarean. Methods 76 parturients with cesarean who suffered chills after CSEA were divided into butorphanol group and tramadol group according to different treatment method, with 38 cases in each group. The clinical effect, adverse reactions, changes of Sp O2 and MAP before medication, 5 min and 15 min after medication were compared. Results The total effective rate of butorphanol group was94.74%, higher than 84.21% of tramadol group, without statistical difference(P〉0.05). MAP in two groups before and after medication had no statistical difference(P〉0.05), Sp O2 5 min and 15 min after medication in butorphanol group had statistical difference(P〈0.05)compared with pre-medication. The adverse reactions rate of butorphanol group was 2.63%, significantly lower than 31.58% of tramadol group, with statistical difference(P〈0.05). Conclusions For parturients with cesarean, the effect of butorphanol is better than tramadol in treating chills after combined spinal epidural anesthesia, and has less adverse effects and important clinical value.
