淋巴细胞主动免疫疗法在不明原因复发性流产患者中的应用

摘　　要：目的:探讨淋巴细胞主动免疫疗法治疗不明原因复发性流产患者的疗效。方法:在2012年1月至2013年12月期间对不明原因复发性流产的患者用ELISA法查封闭抗体,将封闭抗体检测结果为阴性的患者随机分成非免疫治疗组和免疫治疗组,随访两组患者至孕满28周经产前筛查无异常作为保胎成功的标准。结果:共有610例复发性流产患者进行了封闭抗体检测,有451例为阴性,阴性率为74%;自愿接受治疗且资料完整的患者有320例,其中非免疫治疗组130例,免疫治疗组190例,保胎成功率分别为71%和83%,两组差异有统计学意义(P<0.01),因胎儿异常需终止妊娠的发生率分别为3.8%和3.7%,两组差异无统计学意义(P>0.05)。结论:封闭抗体缺乏可能是不明原因复发性流产的一个潜在因素,淋巴细胞主动免疫疗法是针对不明原因复发性流产的一种有效安全的保胎方法。Objectives： To investigate the therapeutic effects of lymphocyte active immune therapy on the treatment of patients with unexplained recurrent spontaneous abortion. Methods： Unexplained recurrent spontaneous abortion patients from 2012 January to 2013 December were selected. ELISA was used to block antibodies. The patients with negative result were randomly divided into non - immunotherapy group and immunotherapy group （P 〈0. 01 ）. Two group patients were followed up until 28 gestational weeks and antenatal screening without exception was the criteria of tocolysis success. Results： A total of 610 patients with recurrent spontaneous abortion received blacking antibody detection, 451 negative （74%） ; there were 320 eases that received the treatment voluntarily and had complete data, in which there were 130 eases in non - immunotherapy treatment group and 190 cases in immunothempy group. The success rotes of preventing miscarriage were 71% and 83%, respectively. The difference was statistically significant （P 〉0. 05）. Conclusion：The lack of blocking antibody may be a potential factor of unexplainod recurrent spontaneous abortion. Lymphocyte active immune therapy is a safe and effective method for treating unexplained recurrent spontaneous abortion.
