摘 要:目的 :探讨"医院-社区-家庭"授权教育模式在胰岛素泵治疗患者中的应用效果。方法 :选择2012年2月至2014年3月内分泌科住院使用胰岛素泵治疗的糖尿病患者60例,实施"医院-社区-家庭"授权教育模式,随访3个月、6个月后监测患者空腹血糖、餐后2h血糖值,糖化血红蛋白(Hb Alc)水平及自我护理能力的变化。结果 :实施授权教育后随访3个月、6个月时患者空腹血糖、餐后2h血糖值,糖化血红蛋白(Hb Alc)水平及自我护理能力与入组前相比明显改善,差异均具有统计学意义(P<0.05)。结论 :"医院-社区-家庭"授权教育模式整合了医院和社区的医疗卫生资源,充分发挥了家属参与的协同作用,能提高胰岛素泵治疗患者的自我护理能力,增强患者治疗依从性,改善糖尿病患者的临床代谢指标。Objective:To explore the effect of authorization education mode of"Hospital-community-family"linkage in patients with insulin pump therapy. Methods:Sixty patients with diabetes using the insulin pump therapy from 2012 February to 2014 March in Department of Endocrinology received the"hospital-community-family"trinity authorized education mode, and then the changes of fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin (HbAlc) level and self-care ability were measured at 3 and 6 months after follow-up. Results:The implementation of authorized education after 3 and 6 months, the fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin (HbAlc) level and self-care ability were significantly improved compared with before, the differences were statistically significant (P〈0.05). Conclusion:The authorized education mode of"hospital-community-family"linkage integrates the health resource of hospital and the community, and gives full play to synergy of family members. This mode can improve the self-care ability of patients with diabetes, increase adherence to treatment of patients, and improve the clinical metabolic parameters in patients with diabetes mellitus.
