中医药联合化疗治疗中晚期大肠癌临床观察及临床研究类中文文献分析  

Clinical Observation on Treating Advanced Colorectal Cancer by TCM Combined with Chemotherapy and Clinical Trials Literature Analysis

摘　　要：目的:总结2004—2014年间中医药联合化疗治疗中晚期大肠癌疗效。方法:搜索2004—2014年国内公开发表的中医药联合化疗治疗中晚期大肠癌临床观察及临床研究类文献,按纳入排除标准筛选文献,通过疗效指标对疗效进行分析。结果:共检索到中医药联合化疗治疗中晚期大肠癌临床观察及研究类文献150篇,符合纳入标准者62篇,其中临床对照观察类文献39篇,占62%,临床对照研究类文献23篇,占38%。有中医证型诊断标准及中医证候积分对比者12篇,占19%;对照文献说明组间基线均衡者42篇,占68%;无一篇文献行样本含量的估算。以WHO标准判定标准的有29篇,占47%,有效率占38%;49篇评价化疗不良反应,占79%;14篇分析中医症候,占23%;49篇以生活质量为评价标准,占79%。结论:中医药治疗中晚期大肠癌在改善症状,减毒增效方面具有一定疗效,并能改善患者免疫功能,延长生存期。但由于缺乏大样本、多中心的随机对照研究,因此,中医药联合化疗在治疗中晚期大肠癌方面的疗效有待进一步论证和研究。Objective：To summarize curative effects of TCM combined with chemotherapy in the treatment of advanced colorectal cancer from 2004 to 2014.Methods： The papers on clinical observation and clinical trials of TCM combined with chemotherapy in treating advanced colorectal cancer were searched, which were published from 2004 to 2014 in China, the literature was screened according to inclusion exclusion criteria, clinical effects were analyzed by observing the indexes. Results： All 150 articles of clinical observation and studies about TCM combined with chemotherapy in treating advanced colorectal cancer were obtained, 62 pieces met the criteria, a-mong them, there were 39 pieces about clinical observation, they occupied 62%, and 23 pieces about clinical trials which accounted for 38%. There were 12 pieces （19%） describing TCM pattern diagnostic standard and the compari-son of TCM syndrome integral;42 pieces （68%） demonstrating the baseline equilibrium between the groups accord-ing to the literature;but there was no article estimating the sample size. There were 29 papers with therapeutic stan-dard came from WHO criteria, they reached 47%, effective rate 38%;49 pieces evaluating adverse reaction of the chemotherapy and occupied 79%;14 papers analyzing TCM syndrome and accounted for 23%;49 papers taking life quality as evaluating standard and they held 79%. Conclusion：TCM in the treatment of advanced colorectal cancer possesses certain effects in improving symptoms, reducing toxicity and increasing effects, the therapy could improve the immunity and prolong survival. But because of the lack of large sample, multicenter randomized controlled study, clinical effects of TCM combined with chemotherapy in treating advanced colorectal cancer need to be further verified and studied.
