摘  要:目的:研究七氟烷后处理对脂多糖(lipopolysaccharide, LPS)诱导的急性损伤大鼠肺组织Toll样受体4(Toll-like receptor 4, TLR4)表达的影响。方法:健康雄性SD大鼠24只,体质量230-280 g,采用随机数字表法,将大鼠随机分为4组(n=6):生理盐水组(C组)、生理盐水七氟烷组(S组)、LPS组(L组)、LPS七氟烷组(LS组)。 C组和L组分别于气管内滴注生理盐水1 mL/kg或LPS 5 mg/kg,S组与LS组分别于气道内滴注LPS 4 h吸入2.4%七氟烷1 h,6 h后处死大鼠取肺组织,观察肺组织病理改变,采用逆转录-聚合酶链反应(reverse transcriptase polymerase chain reaction, RT-PCR)检测肺组织中TLR4 mRNA的表达水平;采用免疫组织化学法检测TLR4的蛋白表达;采用酶联免疫吸附试验(enzyme-linked immunosorbent assay, ELISA)检测支气管肺泡灌洗液中肿瘤坏死因子α(tumor necrosis factor-α, TNF-α)和白细胞介素-1β(interleukin 1 beta, IL-1β)的质量浓度。结果:与C组比较,L组和LS组肺泡正常结构严重破坏,TLR4 mRNA及蛋白表达明显上调,支气管肺泡灌洗液中TNF-α和IL-1β质量浓度明显升高(P<0.01);与L组比较,LS组肺泡结构破坏较轻,且各指标水平明显降低(P<0.01或P<0.05)。结论:七氟烷后处理可以减轻LPS诱导的急性肺损伤,其机制可能与抑制肺组织TLR4表达、减轻炎性反应有关。Objective: To observe the effects of sevoflurane postconditioning on the expression of Toll-like receptor 4 (TLR4) in rats with acute lung injury induced by lipopolysaccharide(LPS). Methods: Twenty-four adult male Sprague-Dawley rats, weighing 230-280 g, were randomly assigned into 4 groups(n=6 each):normal saline group(group C), group(NS+sevoflurane) (group S), group LPS(group L), group(LPS+sevoflurane)(group LS). Rats received intratracheal instillation of normal saline 1 mL/kg or LPS 5 mg/kg respectively. Group S and group LS, 4 h after LPS intratracheal instillation(i.e.after the occurrence of acute lung injury), rats received 2.4% sevoflurane inhalation for 1 h respectively. The rats was sacrificed at 6 h after administration of LPS or NS, and lungs were removed for microscopic examination and for determination of TLR4 mRNA and protein expression in lung tissues, by using reverse transcriptase polymerase chain reaction(RT-PCR) and immunohistochemistry in all groups. Concentration of tumor necrosis factor-α(TNF-α), interleukin l beta(IL-1β) in bronchoalveolar lavage fluid were measured by enzyme-linked immunosorbent assay ( ELISA ) . Results: Compared with group C , lung tissue damage , the expression of TLR4 mRNA and protein and concentration of TNF-α, IL-1β in bronchoalveolar lavage fluid were obviously increased in group L and group LS(P〈0.01). Compared with group L, the parameters mentioned above were significantly decreased in group LS(P〈0.01 or P〈0.05). Conclusion:Sevoflurane postconditioning could reduce LPS-induced acute lung injury in rats through inhibiting the up-regulation of TLR4 expression in lung tissues and reducing the inflammatory response.
