摘  要:目的研究神经节消融对乙酰胆碱(Ach)诱导房颤的碎裂电位时空稳定性的影响。方法 Ach应用于10只犬,在应用前后,测量5个心房位点的不应期及房颤诱发率。于脂肪垫消融前后,分析Ach诱发出的前2次房颤,计算记录到碎裂电位的点数和碎裂电位持续时间。结果 Ach缩短平均各点不应期[(1 38±14)vs(100±15)ms,t=5.86,P<0.01)],脂肪垫消融增加平均各点不应期[(100±15)vs(115±14)ms,t=2.31,P<0.05)。Ach应用后房颤诱发率增加[(76±9)%vs(4±6)%,t=29.77,P<0.01],脂肪垫消融后减少[(76±9)%vs(54±6)%,t=9.10,P<0.01]。记录到碎裂电位的点占全部记录点的百分比于脂肪垫消融后减少(24%vs 82%),并且脂肪垫消融减少碎裂电位的持续时间[(188±63)vs(139±31)s,t=3.12,P<0.05]。结论选择性脂肪垫消融可影响犬Ach诱导房颤碎裂电位的时空稳定性。[ Objective] To investigate the effect of selective fat pad ablation on the spatial and temporal stability of complex fractionated atrial electrogram (CFAE) in acetyleholine (ACh)-indnced atrial fibrillation (AF). [Methods] Aeb was applied in 10 dogs. Effective refractory periods (ERPs) at 5 different sites, AF inducibility and AF duration were obtained before and after fat pad ablation. During the first two induced AF, the number of sites recorded continuous CFAE and the duration of continuous CFAE at every site were measured before and after fat pad ablation.[Results] The average ERP was shortened by Ach application [(138±14) ms vs (100±15) ms, t=5.86, P〈0.001] and increased after fat pad ablation [(100±15) ms vs (115±14) ms, t=2.31, P〈0.001]. AF inducibility [(76±9)% vs (4±6)%, t=29.77, P〈0.001)] increased after Ach application compared to baseline, while fat pad ablation reduced AF inducibility [(76±9)% vs (54±6)%, t=16,P〈0.01]. Percentage of sites with continuous CFAE decreased after fat pad ablation compared to that before fat pad ablation (24% vs 82%). Fat pad ablation reduced duration of continuous CFAE on average [(188±63) vs (139±31) s, t=3.12, P〈0.05)].[Conelusion] Selective fat pad ablation can affect the spatial and temporal stability of CFAE in ACh-induced atrial fibrillation in dogs.
