定喘汤联合西药治疗支气管哮喘系统综述与Meta分析

摘　　要：[背景]定喘汤联合西药治疗支气管哮喘疗效确切。[目的]评价定喘汤联合西药治疗支气管哮喘疗效与安全性。[检索策略]主题词与自由词结合检索CNKI、VIP、Wan Fang Data等数据库,检索时间:建库至2015年3月。同时追溯纳入文献参考文献。[纳入标准]中文随机对照试验(RCT)文献。1研究对象:符合诊断标准,不限年龄和性别,是否采用盲法及有无退出、失访。2干预措施:试验组:定喘汤及加减联合西药;对照组:西药。3结局指标:有效率、不良反应发生率、肺功能等。[数据收集与分析]2名评价员独立对纳入文献进行数据提取,意见不一致双方讨论或第3位评价员协助解决。采用资料提取表,提取内容:样本量、性别、年龄、疗程、干预措施、结局指标等。[主要结果]初检相关文献2047篇,经逐层筛选,最终纳入文献18个,共1600例研究对象。1急性发作期:合并效应量OR值=3.46[95%CI(2.39,4.99)],总体效应检验Z=6.61,P<0.00001。2慢性缓解期:合并效应量OR值=3.49[95%CI(1.46,8.36)],总体效应检验Z=2.81,P=0.005。Meta分析显示:试验组疗效优于对照组。[结论]定喘汤联合西药治疗支气管哮喘具有一定疗效,优于单纯西药。[问题与展望]需要大样本量,高质量的随机对照试验进一步证实。[Background] Shall be dingchuan decoction combined western medicine treating bronchial asthma with curative effect. [Objective] To evaluate dingchuan decoction combined western medicine treating bronchial asthma with curative effect and security. [Retrieval tactics] Subject combined with a free word searching CNKI, VIP, Wan Fang Data such as database, retrieval time： build library until March 2015. At the same time back into the literature references. [Into standard] Randomized controlled trial（RCT） in Chinese literature.（1） the object of study, in line with the diagnostic criteria, no age and gender, whether by blind method, and presence of exit, lost to follow-up.（2） interventions： experimental group： dingchuan decoction and add and subtract in combination with western medicine; Control group： the western medicine.（3） the ending index： efficient and incidence of adverse reactions, such as lung function. Evaluator/data collection and analysis of two independently to data extraction into standard literature, in case of disagreement, discussed by both parties or third part evaluation to help to solve. Content： the extracted with the use of data extraction table, sample size, gender, age, course of treatment, intervention, outcome indicators, etc. [Main results] 2047 early detection related literature, after screening step by step, finally into 18 research literature, and a total of 1600 cases of the object of study.（1） acute phase： consolidation effect quantity OR value = 3.46 [95% CI（2.39, 2.39）], the overall effect inspection Z = 6.61, P〈0.00001）. Meta analysis showed that： dingchuan decoction treatment group is superior to western medicine treatment group.（2） chronic remission： consolidation effect quantity OR value = 3.49 [95% CI（1.46, 1.46）], the overall effect inspection Z = 2.81, P = 0.005）. Meta analysis showed that： the experimental group curative effect is better than the control group. [Problems and prospect] Need large sample size, high quality
